Hemorrhage remote from the surgical site after transsphenoidal approach is uncommon. We report a rare case of epidural hematoma occurrence following pituitary tumor surgery via transsphenoidal approach. During the operation, the arachnoid membrane seemed not to be disturbed but clear fluid like cerebrospinal fluid(CSF) was suspected to be being leaked. Lumbar drain was inserted immediately after surgery. Four hours after surgery, the patient’s consciousness fell to drowsiness and exhibited right hemiparesis. Brain CT scan revealed large amount of hematoma in the epidural space of left parietal area. Emergency craniotomy was performed and hematoma was removed. Over-drainage of CSF through the lumbar drain seems to be major cause for this postoperative complication.
