안와벽을 침범한 Meningioma en Plaque의 수술적 절제 후 반으로 쪼갠 두개골 이식을 이용한 안와벽의 재건

Meningioma en Plaque on Orbital Wall: Reconstruction with Bihalved Calvarial Bone Graft

Meningioma en plaque invading adjacent orbital wall and skull base bone is characterized by hyperostosis of affected bone tissue. Resection of involved superior and inferior orbital wall is inevitable to ablate tumor and there remains reconstruction of removed orbital wall. This case shows how meningioma en plaque resection on sphenoorbital area and reconstruction with bihalved calvarial bone were done. A 47-year-old female patient complaining proptosis visited neurosurgery outpatient clinic. Preoperative computed tomography reveals that a 5cm tumor located at the anterior portion and sphenoid greater wing of the middle cranial fossa. Tumor was diagnosed as meningioma en plaque by characteristic hyperostosis of affected bone. Surgical resection involving the superior and lateral orbital wall was done and the orbital wall was reconstructed with 6x4cm autogenous bihalved calvarial bone graft. Reconstruction was successful without periorbital bony disfigurement and tumor recurrence was not observed during seven years follow up period.
