To resect of the tumors involving the petroclival region in the cerebellopontine angle and ventral to the brain stem have been a surgical challenge for many years. Multiple cranial nerves and blood vessels may be intimately involved with the tumor. We present the case of a 50 year-old female presenting with symptomatic petroclival meningioma treated by transcrusal approach. Hearing in the operated ear improved within 5months after surgery. We conclude that transcrusal approach is a safe and effective approach to the petroclival region. This approach affords the best chance of hearing preservation and an opportunity for maximum tumor resection.
