Clinical symptoms and audiovestibular findings of large and giant vestibular schwannoma

Background : Large and giant vestibular schwannomas (VSs) are considered a challenging and dangerous disease entity owing to their surgical difficulty and following morbidity. The purpose of this study is aimed to identify the clinical symptoms and audio-vestibular findings in patients with large and giant VS. Methods : We retrospectively enrolled 19 patients who were diagnosed with large (≥3 cm and <4 cm) and giant (≥4 cm) VS from March 2005 to February 2018. We studied the clinical presentations and audio-vestibular findings in patients with large and giant VS. Results : There were 7 large and 12 giant VS in our series. Dizziness was the most frequent presenting symptom in 14 patients with large and giant VS. Gradual hearing loss was observed in nearly half of patients who had central symptoms. While 4 patients showed class A in AAO-HNS hearing classification, all patients showed abnormal canal paresis on caloric test. There was no difference about the clinical symptoms and audiovestibular findings between large and giant VS. Conclusions : If we pay attention to the auditory symptoms of large and giant VS, it would be possible to promote early diagnosis of VS.
