Background : Translabyrinthine vestibular schwannoma resection (TLVSR) is rarely performed by an otolaryngologist in Korea. The purpose of this study was to analyze an early experience of TLVSR performed by an otolaryngologist. Methods : Among 47 consecutive patients diagnosed with vestibular schwannoma, medical records in five patients who underwent TLVSR by an otolaryngologist without support of neurosurgeons were retrospectively analyzed. Demographics, clinical features, intraoperative findings and postoperative complications were evaluated. Results : Most patients (80%) presented with sudden sensorineural hearing loss in the affected side. Three of the 5 patients had vestibular schwannoma involving the internal auditory canal and the cerebellopontine angle and the other 2 had intrameatal tumor only. Mean size of the extrameatal portion of the tumor was 17.1 mm. Complete removal was achieved in all 5 patients. Four (80%) of 5 patients with vestibular schwannoma had postoperative normal facial function, but the other had HB grade IV facial function. Cutaneous cerebrospinal fluid leakage was observed in one patient, which was treated with conservative management. Conclusions : Although this study conducted a small number of subjects and analyzed an early experience of TLVSR, an otolaryngologist is able to safely perform TLVSR without aid of neurosurgeons.
