Cerebellopontine angle meningiomas presenting otologic symptoms

Background : Cerebellopontine angle (CPA) tumors produce a complex pattern of symptoms that could greatly delay and confuse a diagnosis. This study evaluated CPA meningiomas presenting with otologic symptoms and reports unusual cases of CPA meningiomas. Methods : We studied the patients presenting with otologic symptoms, such as hearing loss, dizziness, and tinnitus among CPA meningioma patients. We excluded the cases showing only central signs, such as ataxia, seizures, and headache. Results : Ten cases of CPA meningioma presented with otologic symptoms. Four of these cases presented with hearing loss on the lesion side; of these, one presented with sudden hearing loss. Three CPA meningioma patients complained of tinnitus. Eight CPA meningioma cases presented with dizziness. The dizziness pattern of most patients was consistent with disequilibrium, and only one case presented with recurrent vertigo mimicking M ni re’s disease. On magnetic resonance imaging, one case mimicked a vestibular schwannoma. Audiometry and vestibular function tests showed variable results. Conclusions : CPA meningiomas may present with various otologic symptoms and can be misdiagnosed as other disorders. Given the remarkable variability in the manifestations of CPA lesions, we should consider the possibility of a CPA meningioma in addition to acoustic neuroma.
