의철학의 연구 대상과 연구 방법

The Subject and Method of Philosophy of Medicine

의철학이 학문의 세계에서 하나의 독립된 학문으로 인정받기 위해서는 독자적인 연구 대상과 연구 방법을 가져야 한다. 그렇지 못하면 정체성의 위기에 직면하게 되는데 의학의 경우도 그 초창기에 치열한 정체성 논쟁을 겪은 바 있다. 2500년 전 히포크라테스 학파는 우주론적 사변성으로부터 의학을 독립적인 학문으로 지켜가기 위해 전통 의학의 고유한 원리와 방법을 내세우며 의학은 우연한 선물이 아니라 경험, 관찰, 추론과 같은 인과적 지식으로 발견한 진정한 테크네라고 강변했다. 이런 상황은 고대 의학에 관하여 라는 논문에 잘 나타나 있는데 이런 역사적 경험은 학문의 역사가 일천해서 아직 고유한 연구 주제와 방법이 정립되지 않은 의철학에 좋은 교훈이 될 수 있다. 그렇지 않으면 의철학은 인접 학문들의 틈새에서 신분증조차 발급받지 못하는 상황을 맞을 수도 있다. 그런데 의철학이 독자적인 연구 주제와 방법을 모색해가며 자기 정체성을 확립해가는 과정에서 각별히 유의해야 할 것은 의료윤리학과의 관계이다. 의철학은 연구 대상과 연구 방법에서 의료윤리학과 유사성이 많기 때문에 일정한 차별성을 입증할 수 있어야 한다. 그동안 의료윤리학이 의학의 발전에 따른 문제를 윤리적으로 성찰하는 학문으로서 의학의 현장에서 발생하는 생식의 윤리, 죽음의 윤리, 진료의 윤리 등을 다루어 왔다면 의철학은 의학의 본질에 대해 철학적으로 성찰하는 학문으로 의학의 본질을 이루는 의학의 목적, 주체, 원칙, 한계 등을 주로 다루게 될 것이다. 방법론적 측면에서는 의료윤리학의 경우 비참과 칠드레스의 4대 원리처럼 나름대로의 이론들을 개발하여 활용하고 있으나 의철학은 독자적인 방법론을 암중모색하고 있는 상황이다. 이에 본 연구에서는 우리 사회에서 의철학적 연구의 대상이 될 수 있는 것들을 주제들을 정리해보았고 연구 방법으로는 인본주의, 전일주의, 자연주의, 과학주의라는 네 가지 원리를 조심스럽게 예시해보았다. 그런데 서양 의학사에서 의학의 정신과 윤리를 가장 잘 담고 있는 ｢선서｣는 현재 우리 사회가 안고 있는 과제들을 해결하는데 참신한 영감을 줄 수 있다는 점에서 의철학적 논의의 출발점으로 삼을 만하다.

The philosophy of medicine needs its distinctive subjects and methods to avoid the crisis of identity in the world of science. Western medicine, too, had to answer a similar question in the fifth century B.C. At a time the boundaries between disciplines were not sharply defined, the authors of Hippocratic school tried to establish independence of medicine. Especially, the author of On the Ancient Medicine(De prisca medicina) attacked a group of thinkers who attempted to base medical practice on theories drawn from natural philosophy and insisted that medicine was a genuine art(techn ) that had been discovered by following a systematic principle(arch ) and method(hodos); its discovery was the result of observation( skepsios), experience(empeiria) and reasoning(logismos), not chance( tych ). Nowadays the philosophy of medicine, too, faced such a problem, therefore it must discover its peculiar subjects and proper methodology. Then it will encounter medical ethics or biomedical ethics in the process developing its own subjects and methods. They actually share a good deal of affinities and differences. Medical ethics has dealt with reproductive ethics, ethics of death and ethics of treatment; the philosophy of medicine is mainly concerned with the subject, object, aim, method and limitation of medicine. It is beneficial to consider humanism, holism, naturalism and scientism as four principles of the philosophy of medicine. The Oath is a classical text that is composed of philosophical contents and ethical issues. The Oath will give the philosophy of medicine theoretical foundation it needs to qualify as a genuine science.
