의학의 세 차원

Three Dimensions of Medicine: Natural Medicine, Social Medicine and Humanistic Medicine

21세기 초 문명사회를 살아가는 사람들은 대부분 의학을 과학의 한 분야로 받아들인다. 그리고 과학인 의학이 인류를 모든 질병의 고통에서 구원해 줄 것이라고 믿는다. 하지만 의학은 본질적으로 순수한 과학일 수도 없고 그렇다고 예술이나 종교처럼 우리를 실존적으로 구원해주지도 못한다. 인간의 구원이 의학의 목적일 수는 있어도 의학이 바로 구원인 것도 아니고 구원을 약속해 주는 것도 아니기 때문이다. 이 글에서는 과학을 중심으로 한 지금의 의학을 비판적으로 검토하고 의학에 대한 새로운 이해의 틀을 제시한다. 그리하여 의학을 “질병경험에 대한 인간의 종합적 행위체계”로 재규정한다. 그리고 그 경험과 행위를 자연, 사회, 인문의 상호의존적인 세 차원에서 검토하고 각각의 영역을 자연의학, 사회의학, 인문의학으로 명명한다. 이 셋 중에서도 인문의학은 최근에 와서야 관심을 받기 시작했지만 의학의 전체 흐름을 바꿀 큰 잠재력을 가진 영역이다. 인문학은 의학을 연구의 대상으로 삼을 수도 있고 의학의 도구가 될 수도 있으며 의학에 통합될 수도 있다.

Most of us living in the 21th century take medicine as a field of science, and believe that science will save us all from the suffering of diseases. I argue, on the contrary, that medicine cannot be a pure science nor save us. Salvation may be the aim of medicine, but medicine in itself cannot be a savior nor promise to be one. In this paper I review the current biomedicine critically and propose a new way of understanding and constructing medicine. I re-conceptualize medicine as “a composite human action system on the experience of illnesses.” And I review the composite matrix of experience-and-action in three dimensions of Nature, Society and Humanity. The three perspectives of medicine comprise natural, social, and humanistic medicines respectively. Of the three, humanistic medicine is the one that has not been highlighted until recently, but has the potential to change the whole landscape of medicine. The humanities can make medicine as a study subject, can be an instrument of medicine, or can be incorporated into medicine.
