의사의 직업전문성과『히포크라테스 선서』

Physician’s Professionalism and Hippocratic Oath

의학은 신학;법학과 함께 중세 대학의 3대 박사과정 과목으로 인정될 만큼 일찍부터 전문성을 인정받아 왔다. 오늘날에도 의사;성직자;법조인과 같은 전문직 종사자들은 일반직 종사자보다 대체로 높은 수준의 전문성을 갖추고 있고;상대적으로 높은 사회적 지위와 경제적 보상을 받고 있다. 대신에 전문가들은 자신들이 가진 전문성을 자신들을 위해서가 아니라 그 전문성을 필요로 하는 사람들의 이익을 위해 사용하도록 요청받아 왔다. 사익(私益)을 넘어 공익(公益)을 우선하는 자세가 전문직업의식의 출발점이 되고 일반인이 전문직 종사자들에게 보내는 존경과 신뢰의 근거가 된다. 전문가 집단은 자신들의 직업적 전문성을 유지하고 사회적 의무를 이행하기 위해 자율적으로 동업단체를 조직하여 회원들이 준수해야 하는 ‘윤리선언’;‘윤리강령’;‘윤리지침’ 등을 제정하여 실천한다. 이와 같은 전형을 우리는 의사 전문직에서 찾아 볼 수 있다. 오늘날 다양한 분야의 전문직 종사자들이 자신들의 직업적 정체성을 확인하고 자신들이 추구하는 가치와 이상을 담은 선서를 하는데;전문직업인 선서의 기원은 『히포크라테스 선서』이다. 이 선서는 히포크라테스 의학을 의술의 전문성에 맞는 직업윤리까지 갖추게 함으로써 서양의학의 진정한 출발점이 되게 했다. 오늘날 의사의 직업정체성 교육에 의사라는 ‘직업의 정체성’;‘의사의 본분’;‘의사의 이상’에 관한 내용을 포함시키는 것이 바람직하다. 그런데 다행히 『선서』에는 의사의 직업정체성 교육에 관한 내용이 잘 담겨 있다. 그래서 『선서』는 오늘날 의사의 직업전문성 교육에 효과적으로 사용될 수 있을 만큼 논리적이고 체계적으로 작성되어 있다. 의사가 자신의 직업적 정체성을 확인하고 직업적 삶에 대한 반성을 통해 전문가의 위상에 합당한 직업정신과 윤리의식을 지닌다면 의사의 직업 만족도는 지금보다 훨씬 높아질 수 있을 것이다.

The purpose of this article is to look at the relationship between physician's professionalism and Hippocratic Oath. Professional education for medical students had better contain main three issues: physician's identity;physician's duty and physician's ideal. Hippocratic Oath is not an encyclopedic exploration of ancient medicine;but it is important in that ideas of ideals and principles in the medicine are embodies in its words. The Oath does not discuss all issues in the medical professionalism. But the Oath's messages still ring true. Even though the Oath is not the standard for good doctoring;it is a standard. The Oath contains three questions and answers: Who are physicians? What do physicians do? What do physicians pursue? Thus;the Oath can serve as a coherent framework for a curriculum in professional education. It need not be a procrustean bed for a medical curriculum. There can be debates about the moral authority of the Oath in relation to the medical professionalism. But the Oath can serve as an attractive and useful framework for a modern medical professionalism rather than simply being relegated to ritualistic use. The Oath takers are aware of and affirm a venerable tradition of social and professional responsibility. The Oath can play a important role in the development of medical professionalism and self-perception.
