치의학 교육의 변화와 혁신에 관한 인문학적 성찰

Humanistic Reflections on the Change and Innovation of Dental Education

1995년에 미국 의학한림원(Institute of Medicine)은 치의학 교육에 관한 백서 『기로에 선 치의학 교육: 도전과 변화』를 펴내고 치의학 교육의 전반적인 변화를 촉구했다. 2005년에 미국치의학교육학회는 광범위한 의견 수렴 과정을 거쳐 변화와 개혁을 위해 추구해야 할 가치와 구체적인 방향 그리고 공중의 구강보건 요구에 부합하는 치과의사의 기준을 제시했다. 이 논문은 그 기준과 방식들을 한국의 상황에 적용하고자 할 때 생길 수 있는 문제들을 검토하고 대안을 제시하기 위한 것이다. 위의 두 문서는 ① 주류 의학에서의 소외와 ② 기초과학과 임상치의학의 괴리를 지금 치의학이 당면한 주요 문제로 진단한다. 변화와 혁신은 이 소외와 괴리를 극복하는 방향을 취해야 하는데 그 과정에서 가장 중요한 가치는 비판적 사유이다. 이 문서들은 기존의 가치와 관행 그리고 교수자와 학습자 자신들의 생각과 관습을 비판적으로 재검토하기를 요구한다. 이 과정에서 길을 잃지 않기 위해서는 반드시 지켜야 할 중심 가치와 원칙을 가져야 한다. 필자는 그 중심 가치로 역량, 진정성, 책무성을, 의료 전문인 일반이 추구해야 할 목표로 "진료 역량과 공감 능력을 갖춘 학습자이고 지도자이며 과학자인 의료인"을 제시한다. 이 가치와 일반 목표는 "학술적 분위기에서 생명의학 훈련을 받았으며 의사소통 능력이 뛰어난, 포괄적 증거기반치의학을 시술하는 치과의사"라는 구체적 목표로 수렴된다. 마지막으로 변화는 선택이 아니라 치의학 교육의 철학이어야 함을 강조한다. 교육의 변화와 혁신은 교육기관, 교수자, 학습자 모두가 변화하려는 의지를 가져야만 가능하기 때문이다. 이 논문이 그런 의지와 폭넓은 의견 수렴을 추동하는 작은 불씨가 되기를 희망한다.

Many people talk about 'Dental Education at the Crossroad' at least since 1995 IOM report of the same title. ADEA CCI in 2005 took the initiative in facilitating transformation of dental education so that the students graduate with competences required to meet the oral health needs of the public through the 21st century. In this paper I briefly review the history of dental education in America and discuss the implications when applied in Korean context. Following the IOM Report, I identify problems in the inappropriate relationships between medicine and dentistry as well as basic and applied sciences in dentistry. In order to solve these problems we need to think critically about the current institutional customs and our own selves. These procedures need to be guided by principles and values. I propose core values, purposes and goals of health professional education in general and in dentistry in particular, which will guide us in the process of change and innovation. The core values that I propose are Competence, Integrity and Accountability. Goals of health professional education are Competent, Empathetic Life-long-learners, Leaders and Scientists(CELLS). These values and goals converge on Academically educated, Bio-medically trained, Communicatively proficient Dentists who practice Evidence-based comprehensive dentistry(ABCDE). Finally I emphasize that changes are not a matter of choice but a word of philosophy because education is essentially making changes in both faculties and students.
