의학적 내러티브의 심리적 구조

The Psychological Structure of Medical Narrative

이 글은 의학의 영역에서 내러티브 연구가 어떤 의미와 성과를 거두었는지를 살펴보고, 언어심리학에 기초하여 의학적 내러티브 분석을 위한 하나의 시론을 서술하는 데 목적이 있다. 의학적 내러티브 이론의 대표자들인 리타 샤론, 캐슬린 몽고메리 헌터, 아서 프랭크의 작업은 의학적 내러티브의 중요성과 구조, 그것의 윤리적 의미를 밝히는 데 중요한 기여를 한 것으로 평가할 수 있다. 하지만 그들의 이론은 의학적 내러티브의 언어심리학적 구조에 대한 연구와는 거리가 있다. 내러티브는 언어적 정보 이외에 인간의 무의식과 심리적 세계를 담고 있다. 내러티브에서 중요한 것은 내용이 ‘무엇’이냐가 아니다. 그보다 더 중요한 것은 내용이 ‘어떻게’ 서술되어 있는가이다. 왜냐하면 내러티브의 내용은 화자의 의도와 무의식을 완전히 드러내는데 한계가 있기 때문이다. 화자의 의도와 무의식은 내러티브의 내용을 창조하는 ‘방법’을 통해 전달된다. 물론 리타 샤론과 헌터도 이점에 주목하고 있는 것은 분명하다. 그들이 내러티브를 분석하면서 플롯, 시간, 성격 등의 개념을 도입하고 있는 것은 바로 이러한 이유 때문이다. 하지만 내러티브가 어떻게 서술되어 있는가 하는 문제는 본질적으로 언어의 구조, 의미와 밀접한 관계가 있다. 스타니슬라프스키에 따르면 언어로 표현된 내러티브의 배후에는 내적 의미가 존재한다. 다시 말해 내러티브 속에 담긴 생각이나 사상은 기호에 의해 전달될 뿐만 아니라 그것의 내적 의미에 의해 매개되는 것이다. 이 내적 의미는 화자의 성향, 감정, 요구, 의지 등이 반영된 것이다. 환자의 내러티브 속에 감춰진 내적 의미를 제대로 파악하기 위해서는 그것의 ‘내적 어조’를 파악하는 것이 중요하다. 플라톤이 주장한 자유인 의사의 진료 방법에서 ‘관찰’, ‘상담’, ‘설득’, ‘동의’ 등은 내러티브 의학에서 강조하는 ‘듣기’, ‘말하기’, ‘설득하기’ 등과 유사하다. 이렇게 보면 최근에 주목받기 시작한 내러티브 의학의 역사는 꽤 오래된 것이라고 할 수 있다. 요컨대 내러티브 의학이 없었던 것이 아니라 단지 우리가 그것을 잊고 있었던 것이다.

The purpose of this article is to describe the meaning and achievements of narrative research and to present a method for the medical narrative analysis based on psycholinguistics. The works of Rita Charon, Montgomery Hunter, Arthur Frank were an important contribution to reveal the structure, and its ethical implications of medical narrative. But their theory is different from the study on the psycholinguistic structure of the medical narrative. The narrative contains the human unconsciousness and the psychological world in addition to verbal information. 'What's content' is not the important thing in the narrative. Even more important is 'how describe contents'. Because the content of the narrative is limited to expose fully the intent and the unconscious of the speaker. The speaker's intention and subconsciousness is passed through the 'how' to create the contents of the narrative. Of course, it is clear that Rita Charon, Hunter also know this. They have introduced the concept of plot, time, character by analyzing the narrative. But how is described the narrative is closely related to the meaning and the structure of the language. According to Stanislavskii there is internal means behind the narrative expressed in the language. In other words, thoughts or ideas contained in the narrative will not only be transmitted by the symbols but also be mediated by its inner meaning. The inner meaning reflects inclination, feelings, needs, intention of the speaker. It is important to understand the 'inner tone' contained in the patient's narrative in order to properly know its hidden meaning.
