서사에 대한 의철학적 비판

Narrative and Philosophy of Medicine: Focusing on the Pros and Cons of Narrative Medicine

서사는 의료인문학의 여러 분야에서 주요한 방법론이자 토대로서 많은 주목을 받고 있다. 서사가 현대 의학에서 소외된 환자의 주관적 측면에 접근할 수 있는 통로로 여겨지기 때문이다. 하지만 서사에 대한 이런 열광은 서사 만능주의에 대한 우려를 낳고 있기도 하다. 의학과 인문학을 연계하려는 많은 작업이 서사에만 관심을 쏟는 것은 결코 바람직한 현상은 아닐 것이다. 이런 점에서 서사에 대한 의철학적 성찰이 무엇보다 필요한 시점이다. 그동안 의철학 영역에서는 서사에 대한 논의가 산발적으로만 이루어졌다. 따라서 본 논문에서는 서사 의학에 대한 찬반 논의를 중심으로 의철학 영역에서 서사에 대한 논의가 필요한 이유와 그 방향에 대해 탐구해 보고자 한다. 우선 그동안 의철학 영역에서 이루어졌던 서사에 대한 논의를 크게 질병 체험과 관련한 현상학과 해석학, 그리고 임상적 추론이나 의사 결정과 연결된 인식론으로 나누어 고찰해 본다. 그리고 미국의 내과 의사인 리타 샤론(Rita Charon)에 의해 제창된 서사 의학을 살펴보면서 임상의학에서 서사를 활용하는 방식에 대해 논의하고, 최근에 서사 의학 분야에서 나타나고 있는 새로운 흐름인 스키마 중심의학과 서사 기반의 근거중심의학에 대해서도 살필 것이다. 또한, 서사 의학에 대한 최근의 반대 움직임을 네 가지 측면 즉, 서사 자체의 한계, 도구주의, 감상주의, 서구 편향 등으로 정리하고 이런 논의를 바탕으로 서사에 대한 의철학적 탐구가 서사 의학에 어떤 방향성을 제시할 수 있는지 탐구해 볼 것이다.

Considered as a useful tool for gaining access to the subjective aspect of marginalized patients in modern medicine, narrative has received much attention as the foundation and principal methodology in various fields of medical humanities. However, this enthusiasm for narrative give rise to concerns about the abuse of narrative. In this regard, it is time to ponder about philosophical meaning of narrative. So far in the areas of philosophy of medicine, the discussion on the role of narrative has been carried out only occasionally. In this paper, I will explore and investigate the reason and the need to discuss the meaning and role of narrative in the areas of philosophy of medicine, focusing on the pros and cons of narrative medicine. First, previous discussions about narrative in the areas of phiosophy of medicine will be categorized as 1) phenomenology and hermeneutics associated with illness experience and 2) epistemology assoicated with clinical reasoning and decision making process. And then I will explore the narrative medicine advocated by the U.S. physician, Rita Charon, to discuss how to take advantage of the narrative in clinical medicine and also investigate new directions of narrative medicine in recent years. The pros and cons of the narrative medicine will be also discussed, especially in terms of the limitations of the narrative itself, sentimentalism, instrumentalism and the western cultural deviation etc. Based on above discussions, you will have insights on the role of the philosophy of medicine and future directions of the narrative medicine.
