질병서사와 치유에 관한 생명-사회-인문학 가설

The Hypothesis of Bio-Socio-Humanities on Illness Narrative and Healing

우리는 음식을 먹고 부모와 이웃의 보살핌을 받으며 경험을 이야기하면서 살아간다. 질병은 대체로 생물학적 ‘현상’이지만 그것을 치유와 묶어서 생각하면 사회적이고 인문적인 ‘경험’이 된다. 서사의학은 인간이 기본적으로 ‘이야기하는 동물’이라는 전제에서 출발한다. 이 글은 그 전제를 받아들여 생물학적 현상과 사회적이고 인문적인 경험이 별개의 사건이 아니며 발생에서부터 긴밀하게 얽혀 함께 진화하는 실재와 경험의 복합체라고 주장한다. 생물학적 활성이 전혀 없는 물질을 투여했는데도 현저한 증상 개선효과를 보이는 플라시보 현상은 서사의학이 임상활동에서 활용할 수 있는 도구 이상이라는 사실을 말해준다. 이 현상은 실재와 경험의 분리가 사실은 개념적 허구일 뿐이라는 합리적 의심을 품게 한다. 논문은, 지금까지 의학 안팎에서 이야기된 질병경험에 대한 또한 의학 자체에 대한 서사를 정리하고, 지금 진행되고 있는 의학교육 개혁을 서사적 전환이라는 큰 흐름 속에서 파악한 후 이 흐름을 담을 수 있는 대안적 담론으로 생명-사회-인문학을 제안한다. 이 담론은 명징한 논리적 분석이 아닌 은유를 통한 이해와 이야기를 통한 실천을 지향하는데, 트렘볼린에서 함께 뛴다는 상상을 통해 그 이해와 실천의 다양한 가능성들을 탐색해 본다.

We live on food as well as on loving care of parents and stories embodying the food and care. Diseases are biological phenomena but also social and human experience when we think them in relation to the favourable progress; i.e. healing. Narrative medicine begins from the premise that humans are basically storytelling animals. This paper accepts the premise and argues that biological phenomena and socio-human experiences are not separate occurrences. They are co-evolving complex of reality and experience. Placebo responses tell us that narrative is much more than a tool for clinical practices. Placebos make us skeptical of the separation of reality and experience and that they are two aspects of the same phenomenon. This paper summarizes the narratives told about illness experiences and medicine itself, and then identify the current reform in medical education as a part of the mainstream of narrative turn. As an alternative to the current biomedical and bio-psycho-social model of health, I propose Bio-Socio-Humanities which embodies the narrative understanding, living, and practice. This discourse does not use clear logical analysis but pursues metaphorical understanding and narrative practices. The image of jumping together on an elastic cloth of trampoline fits well with my assertion.
