약물효과 평가에 대한 인식론적 고찰

An Epistemological Reflection on the Evaluation of Drug Effects

약물요법은 각종 질병을 치료하는 방법의 하나로서 현대의학의 주축이며, 전체 의료비의 지출에서도 상당한 부분을 차지하고 있다. 종전 의학에 비해 괄목할만한 치료적 성과를 보여주고 있으나 약물요법은 그 과정에서 소기의 목적을 달성하기가 용이하지 않다. 약물효과 평가에 어떠한 문제점이 있다면 약물투여에 따른 약물효과를 온전하게 제대로 반영하지 못하거나 약물효과에 대한 그릇된 평가를 가져와서 약물요법의 실패를 초래할 수 있다. 현행 약물요법의 약효평가체계에서는 치료효과와 부작용 및 소요비용과 같은 측정 가능한 정량적 지표가 치중되어 있는 반면, 주관적 측면에 대한 고려가 미흡하다는 문제점이 발견되고 있다. 이밖에도 여러 문제점이 관찰되어 성찰 필요성이 제기됨에도 불구하고 반성적 접근이 부족한 실정이다. 따라서 약물요법에서의 약물효과 및 약물효과 평가 과정에서 발견되는 문제점들에 대한 의철학적 성찰을 시도하였다. 이를 위하여 약물효과와 그 약효평가를 뒷받침하는 기존의 지식체계에 대한 인식론적 고찰이 이루어졌다. 이러한 논의를 토대로 문제점들에 대한 개선 방안에 대해서도 살펴봄으로써 약물요법의 실질적인 진전에 조금이나마 기여하고자 하였다. 현재의 약물에 대한 담론은 과학적인 의학에 편중되어 단조로운 경향을 나타내지만, 본 글에서는 보다 다양한 시각을 담아내어 다채롭게 발전해나가는 데에도 일조하고자 한다.

Drug therapy, one of treatment modalities for diseases, plays a vital role in modern medicine and takes a considerable amount of total health expenditure. Although current drug therapy has demonstrated remarkable therapeutic outcomes compared to those of previous medicine, it is not easy to fulfill its anticipated purposes. If there are problems in evaluating drug effects, the effect of drugs after drug treatment would not be reflected properly or would be assessed wrongly, resulting in the failure of drug therapy. While the evaluation system of drug effects in current drug therapy attaches much attention to measurable quantitative indices such as therapeutic effect, adverse effect and required cost, the consideration of subjective factors is lacking. Despite the necessity of critical introspection raised by several problems in addition to above one, reflective approaches have been to date insufficient. The reflection on the problems found in the evaluation process of drug effects was therefore done in this study using the philosophy of medicine. To this end, the epistemological inquiry into the knowledge system, which underpins evaluating drug effects, was carried out. Based upon the arguments, the improving ways for the problems were discussed to contribute somewhat to substantive betterment of drug therapy. Current discourses on drugs seem to be monotonous because of too much inclination to scientific aspects of medicine and the present study also hopes to play a part for developing m ultifarious drug discourses to encompass a great variety of perspectives.
