뇌졸중환자와 배우자의 재활회복 경험에 관한 질적 연구

A Qualitative Study on the Rehabilitation Recovery Experience of Stroke Patients and Spouses

본 연구는 뇌졸중 환자의 재활동기에 영향을 주는 요인을 확인하기 위해 부부의 면담내용에 기반한 질적 내용분석(Qulitative content analysis)을 적용하였다. 연구 참여자는 서울, 경기도 소재의 100병상 이상의 재활전문병원 4곳에 입원 중인 뇌졸중 환자와 배우자로 총 11쌍(22명)을 개방적 질문을 통한 면담을 진행하였다. 면담 자료의 분석은 Krippendorff(2003)의 내용 분석 방법(content analysis)을 사용하였다. 연구 결과 ｢환자의 우울｣, ｢배우자의 우울과 부담감｣, ｢환자, 배우자의 부부친밀감｣, ｢환자의 재활동기｣의 4가지 영역으로 확인 되었다. ｢환자의 우울｣은 ‘내 마음대로 할 수 없는 내 몸’, ‘고달프고 힘들어진 삶’, ‘다시 찾은 생명’, ‘밀려드는 후회’, ‘공존하는 절망과 희망’의 범주로, ｢배우자의우울과 부담감｣은 ‘부담스러운 현실’, ‘공존하는 절망과 희망’의 범주로, ｢환자, 배우자의 부부친밀감｣ 은 ‘새삼 느낀 고마움...사랑’, ‘끝까지 함께하고픔’, ‘우리 부부의 성’의 범주로, ｢환자의 재활동기｣는‘회복에 대한 희망’의 11개의 범주로 도출되었다. 뇌졸중으로 치료 중인 환자의 회복 과정의 경험 속에서 환자의 재활동기에 영향을 주는 영향요인의 확인으로 이는 뇌졸중 환자의 치료에 중요한 요소로활용될 수 있을 것이다.

This study used the Qualitative Content Analysis based on the interviews of couples to investigate the factors affecting the rehabilitation motivation of stroke patients. The participants in the Qualitative Content Analysis were 11 couples of the stroke patients and their spouses (22 persons in all). The patients were from four rehabilitation hospitals with more than 100 beds each in Seoul and Gyeonggi-do province. The analysis of the interview data was made using Krippendorff’s content analysis (2003). As a result of the study, ｢patient's depression｣, ｢spouse's depression and burden｣, ｢patient-spouse marital intimacy｣, and ｢patient's rehabilitation motivation｣ were classified into 4 areas, and a total of 11 categories were classified. This can be used as an important factor in the treatment of stroke patients by identifying influencing factors that affect the patient's rehabilitation motivation in the experience of the recovery process of the patient being treated for stroke.
