노인요양시설의 COVID-19 집단감염 대응체계 개선방향

Improvement of the COVID-19 Pandemic Response System in Elderly Care Facilities

본 연구에서는 코어시스템(core system) 모델을 이용하여 노인요양시설 COVID-19 집단감염 사례를비교 분석하고 집단감염 발생 시 대응체계의 개선방향을 제시하는 데 목적이 있다. 코어시스템 모델(core system)을 이용하여 가치, 제도, 리더십, 헌신, 전문성의 관점에서 개선방향을 다음과 같이 제시하였다. 첫째, 가치(value) 측면에서 ‘코호트 격리’ 조치에 따른 코호트 단계별 매뉴얼 등을 법적근거로 만들어 노인요양시설 내 입소자와 종사자의 안전권과 생명권이 존중되어야 한다. 둘째, 제도(institution) 측면에서는 노인요양시설의 상황에 맞는 위기관리 매뉴얼이 법적으로 제도화되어야 한다. 셋째, 리더십(leadership) 측면에서 노인요양시설의 경우 중앙정부에서 동일한 기준으로 우선적인재정적 지원이 이루어져야 한다. 넷째, 헌신(devotion/commitment) 측면에서 대체인력을 지원할 수있도록 전담 사회서비스원 활용과 긴급 인건비 지원 등의 보상책을 마련해야 한다. 다섯째, 전문성(expertise) 측면에서 노인요양시설의 보건·감염전문관리사 인력 기준 마련과 지역별 감염관리순회간호사 인력 활용 방법을 모색해야 한다.

The purpose of this study is to compare and analyze cases of COVID-19 group infection in elderly care facilities using the Core System model, and to suggest improvement directions of the response system. Using the Core System, the improvement directions in terms of value, institution, leadership, devotion, and expertise are presented as follows. First, the right to safety and life of residents and workers in elderly care facilities should be respected by creating a cohort step-by-step manual according to the ‘cohort isolation’ measure as a legal basis. Second, crisis management manual suitable for elderly care facilities should be legally institutionalized. Third, the central government should provide financial support based on the same standards. Fourth, compensation measures such as the use of dedicated social service providers and emergency labor support should be prepared so that replacement workers can be supported. Fifth, it is necessary to prepare the personnel standards for health and infection specialists in senior care facilities and to find ways to utilize the personnel of infection control patrol nurses by region.
