북한 사회보험제도와 의료국영화

National Health Service: The Origin, Ideals, and Reality of Free Treatment, 1945-1950

해방 직후 북한의 사회보험제도 실시는 무상치료제도 구현의 시작이었다. 북한지도부는 ‘전면적 무상치료제’를 바로 실시하기 어려운 상황에서 노동자, 사무원과 이들의 부양가족으로 대상을 제한하여 제도를 단계적으로 확대해갔다. ‘사회보험제도에 의한 무상치료제의 실시’는 의료국영화를 수반했다. 보건의료에 활용할 수 있는 자원이 충분하지 않았던 탓이었다. 의료국영을 통해서 의료 공급의 문제를 개선시켜 사회보험을 통해서 조직된 의료 수요를 해결해 나가야 했다. 본 연구는 이러한 차원에서 사회보험제도가 의료국영화와 맞물려가며 무상치료제의 원형을 만들었던 기반을 파악하고자 했다. ‘사회보험제도에 의한 무상치료제의 실시’는 이후 ‘전면적 무상치료제도’로 확대될 단초를 남기고 있었는데, 헌법 조항에서 사회보험제도의 대상을 “공민”으로 규정하여 제도의 적용대상을 확대시키면서 전체 대중을 포괄될 가능성이었다. 다른 하나는 국영병원은 사회보험 적용대상이 아니더라도 사회보험제도의 혜택을 일괄 적용되도록 한 점이었다. 의료국영화가 완료되면, 사회 보험 대상 여부와 관계 없이 사회보험제도의 수혜를 받게 되는 것이었다.

Immediately after the liberation, the implementation of a social insurance system began in North Korea with the introduction of a system of free medical treatment. The North Korean leadership gradually expanded this free treatment system, but limited it to laborers, clerks, and their dependents at a time when it was difficult to adopt a full-scale system of free treatment. Due to a lack of sufficient healthcare resources, this implementation of free treatment was accompanied by the nationalization of medical care, which aimed to solve the lack of medical resources by meeting the demand for organized medical care through the new social insurance system. This study attempts to examine the basis for creating the prototype of the free treatment system in conjunction with the nationalization of medical care. The implementation of North Korea’s free treatment system had a possibility to expand to a “full free treatment system.” First, the entire public could be included in the scope of the system, with the subjects of the social insurance system defined as “citizens” in its constitutional provisions. Second, state-run hospitals were required to offer the benefits of the social insurance system, even if their patients were not part of the system. When the nationalization of medical care became complete, patients would receive the benefits of the social insurance system, regardless of whether they were subject to it.
