COVID-19 mRNA 백신 주사 후 발생한 백내장 2예

목적: COVID-19 mRNA 백신 주사 후 진행한 단안 백내장 2예를 보고하고자 한다. 증례요약: 2명의 환자가 mRNA 백신 후부터 시작된 단안의 시력저하를 주소로 내원하였다. 40세 남자 환자는 mRNA-1273 (ModernaⓇ, Cambridge, UK) 2차 주사 후부터 발생된 좌안 시력저하를 주소 내원하였으며 40세 여성 환자는 BNT162b2 (PfizerⓇ, New York, NY, USA) 3차 주사 후 발생한 양안 시력저하를 주소로 내원하였다. 두 증례 모두 안과적 과거력도 없었으며 안저검사에서 뚜렷한시신경병증이나 망막병증은 없었다. 수정체유화술 및 인공수정체 삽입술을 시행 후 2개월의 최대교정시력은 향상되었고 안압은 안정적으로 유지되었다. 결론: COVID-19 mRNA 백신 주사 후 백내장의 발생 및 진행 가능성을 고려해야 할 것으로 생각한다.

Purpose: To report two cases of cataract that progressed after COVID-19 mRNA vaccination. Case summary: Two patients visited our clinic with decreased visual acuity in the left eye that began after mRNA vaccine. A 40-year-old man visited our hospital with decreased visual acuity in the left eye that developed 1 week after injection of the second dose of mRNA-1273 (Moderna ® ,Cambridge, UK). A 40-year-old woman visited our clinic with decreased visual acuity in the left eye that occurred on day 46 after the injection of the third dose of BNT162b2 (Pfizer ® , New York, NY, USA). Neither case had any relevant ophthalmic history, and there was no clear optic neuropathy or retinopathy on fundus examination. After phacoemulsification and intraocular lens implantation, the best-corrected visual acuity improved, and intraocular pressure has remained stable. Conclusions: The possibility of cataract development or progression after COVID-19 mRNA vaccine injection should be considered.
