노인장기요양보험제도의 사회적 성과에 대한 고찰

A study on the social aspects of the performance of the National Long-term Care Insurance (NLTCI)

본 연구는 노인장기요양보험제도의 사회적 성과부분에 대한 평가를 위해 실시되었다. 노인장기요양제도의 핵심 목표중 하나인 가족의 수발부담 및 삶의 질에 초점을 두어 제도 출범 후 이 기본적 목표 달성에 충실했는가를 살펴보고 서비스의 만족도 등을 전국적으로 실시한 자료를 분석한 최초의 연구가 되겠다. 여러 선행연구들을 통해 제도 자체의 사회적 성과관련 지표를 구성하여, 전국적으로 장기요양서비스를 이용하는 이용자 가족 1000명을 대상으로 자료를 확보하고, 이 결과를 이용서비스 유형별, 등급별 및 의료보장형태별로 살펴보았다. 수발자의 삶의 질, 수발부담, 만족도 및 대체효과 등의 대부분 영역에서 정기요양 제도의 긍정적인 효과가 나타나는 것을 알 수 있었다. 구체적으로 보면 수발자의 85.4%는 삶의 질이 향상되었다고 응답했으며, 신체적 수발부담과 심리적 수발부담에서 각각 84.7%, 92.1%의 응답자가 감소되었다고 응답했다. 본 연구에서는 단순한 수발부담의 감소 뿐 아니라 수발시간의 활용에 대해서도 살펴보았는데 응답자의 75.9%가 가족관계 향상에 사용했다고 응답했으며, 특히 95.8%가 취업 및 경제활동 등에 활용한 것으로 나타나 수발에 사용된 시간의 생산적 활동에 대체된 성과는 높은 것으로 나타났다. 그러나 제도 시행이 지속되면 서비스 이용자의 기대치가 점차 올라 질적 향상이 동반되지 않으면 지속적인 성과의 긍정적 측면을 담보하기 어려울 수가 있다. 질적 향상은 물론 어느 정도의 재원이 요구되는 부분이지만 일단 서비스에 대한 적응 기간이 지나면 보다 양질의 서비스를 요구하는 것이 이용자의 목소리라는 점을 인식하고 질 개선에 더욱 관심을 가져야 하겠다.

This study aims at evaluating several policy outcomes of NLTCI. The study, particularly, focuses on the several outcomes related to the lifestyle and burden of caregivers' based on their experience of NLTCI services. The study consists of long-term care performance outcomes from previous related foreign studies of long-term care. The data of this research was derived from the phone survey of 1000 caregivers whose father or mother uses community-based NLTCI services in their community. The study founds that NLTCI have had positive impacts on quality of life, burdens, satisfactions, and substitute effects of caregiving from most caregivers in this study. In particular, 85.4% of the caregivers answered their quality of life to be enhanced, 84.7% and 92.1%, physical and psychological burden, respectfully. This study also showed the positive substitute effects of caregiving in that 75.9% of the respondents spent more time on enhancing family relationship, 95.8% of the respondents spent more time on job-related or working time. The study, also found out that the more concrete and objective measures to evaluate the exact long-term care policy outcomes.
