This review addresses the physiologic changes of postmenopausal genitalia, as well as a clinical overview of common gynecologic problems in postmenopausal senile women. While these conditions are often managed by a gynecologic specialist, many women may present to the primary caregiver, still reluctant to address their symptoms. The primary caregivers must be aware of the effect of aging and hormonal changes on the urogenital system in postmenopausal women and how these changes may produce symptoms. It is necessary to know and get used to basic gynecologic history taking and physical examination. Practical counseling tools for patients’ self-care, initial treatment guidelines, and indications for referral are also discussed.
