후방유수정체 안내렌즈삽입술에서 파장가변 빛간섭단층촬영계의 안구 생체계측의 효용성

Efficacy of Biometry Using Swept-source Optical Coherence Tomography for Posterior Chamber Phakic Intraocular Lens Implantation

목적: 후방유수정체안내렌즈(implantable collamer lens, ICL)삽입술 시 안정성에 영향을 주는 요소 중 하나는 적절한 ICL의 크기를정하는 것이다. 적절한 크기를 결정하기 위해 Orbscan II와 IOL Master 700으로 측정된 horizontal white to white diameter (WTW)의값을 비교하고, 각각의 장비에 따른 WTW의 경향성과 ICL sizing에 활용 방법을 알아보고자 한다. 대상과 방법: 2019년 1월부터 2020년 2월까지 본원에서 난시용 ICL 삽입술을 시행받고 1년 이상 경과 관찰이 가능했던 45명, 90안을대상으로 의무기록을 후향적으로 조사하였고, 측정 장비별 WTW와 전방깊이의 상관관계를 분석하였다. 결과: IOL Master 700과 Orbscan II로 측정한 평균 WTW는 각각 12.2 ± 0.3 mm, 11.6 ± 0.3 mm였고, 전방깊이는 각각 3.28 ± 0.16 mm, 3.20 ± 0.15 mm였다. IOL Master 700 측정값은 Orbscan II 측정값보다 WTW는 평균 0.57 ± 0.12 mm, 전방깊이는 평균0.08 ± 0.04 mm 더 큰 값을 보였다. IOL Master 700으로 측정한 WTW과 전방깊이는 Orbscan Ⅱ로 측정한 값보다 각각 유의하게크게 측정되고 회귀분석 결과 높은 상관관계를 보였다(R2=0.875, p<0.001) (R2=0.913, p<0.001). 결론: Orbscan II를 이용한 WTW와 같이 IOL Master 700으로 측정한 WTW값도 ICL 크기를 정하는데 단독으로 또는 더불어 참고값으로 활용할 수 있는 지표가 될 수 있다.

Purpose: The proper implantable collamer lens (ICL) size affects ICL stability. This study compared device efficacy using white-to-white diameter (WTW) measurements with Orbscan II and IOL Master 700. Methods: We retrospectively studied 90 eyes (45 patients) who underwent toric ICL implantation from January 2019 to February 2020 and were followed for 1 year. The correlation between WTW and anterior chamber depth (ACD) for each measuring device was analyzed. Results: The mean WTW measured by IOL Master 700 and Orbscan II was 12.2 ± 0.3 and 11.6 ± 0.3 mm, respectively, while the mean ACD was 3.28 ± 0.16 and 3.20 ± 0.15 mm. The WTW and ACD measured with IOL Master 700 averaged 0.57 ± 0.12 and 0.08 ± 0.04 mm larger than with Orbscan II. The differences were significant and the regression analysis had high correlations (R2 = 0.875 and R 2 = 0.913, respectively; both p < 0.001). Conclusions: WTW measured by the IOL Master 700 can be used as a reference either alone or together with the Orbscan II value to determine ICL size.
