Residency Curriculum for Community Psychiatric Practice

1996년 정신보건법이 제정된 이후 국내 정신보건 현황은 빠르게 변화하고 있다. 지역사회내에서 만성정신질환자를 위한 다양한 프로그램을 계획하고 운영할 수 있는 정신과 의사 인력이 매우 부족한 실정이다. 이런 수요를 충족시키기 위해서는 정신과 전공의 과정에서 지역 정신보건사업을 경험하고 훈련시키는 것이 필요하다. 이를 위해 외국에서 시행하고 있는 전공의 지역사회 정신보건교육 프로그램을 살펴보았다. 우선 지역사회 정신의학의 정의를 임상, 자문, 행정, 학문 영역으로 나누어 보았다. 전공의의 지역사회정신보건 프로그램을 통해 도달하여야 할 목표를 지식, 기술, 태도면에서 나누어 살펴보았다. 또한 실제적인 전공의 교육 프로그램을 소개하였고, 그 구성의 특성에 대하여 고찰하였다. 국내 실정에 맞는 전공의 지역사회 정신보건 교육 과정 개발이 시급하다.

Korean mental health situation is changing rapidly after Korean Mental Health Law legislation in 1996. There is a increasing need to recruit and retain more psychiatrists who can plan and implement organized programs with the required skills and interests to work with chronic mentally ill patients in community settings. Residency training program can be a unique solution to meet this need which have the capacity to prepare psychiatrists for community practice. Residency training programs for community psychiatric practice in United States is introduced. A consensus definition of the clinical, administrative, consultative and academic areas of contemporary community psychiatric practice developed by the American Association of Community Psychiatrists is presented. A comprehensive list of knowledge, skill, and attitude objectives are provided as guideline for curriculum development. Structure of clinical training experiences is reviewed. And residency curriculum for community psychiatric practice in Oregon state is exampled. Development of resident training program for community psychiatric practice is urgent in Korea.
