연구목적 : 본 연구는 조현병 환자에서 삶의 질, 낙인, 병식, 우울감 및 조현병 증상의 각변화량 간의 관계에 대해 알아보고자 하였다. 방 법 : 2015년 1월부터 2016년 1월까지 DSM-5을 통해 조현병으로 진단된 30명의 입원환자를 대상으로 삶의질, 낙인, 병식, 우울감, 정신병적 증상의 변화량에 대해SQLS-R4K, KISMI, SUMD-K, CES-D 및 PANSS을 사용하여 입원 당시 및 4주 후 2회 평가하였다. 각 척도의 전후 평가척도는 대응표본 t test를 시행하였고 모든 척도의 총점 변화량, 하위척도 변화량과 삶의 질 변화량에 대해 피어슨 상관분석을 시행하였다. 결 과 : SQLS-R4K 삶의 질 변화량은 CES-D 우울감 변화량과 양의 상관관계를 보였고 SUMD-K 현재시점의 정신장애의 사회적 결과 자각 척도 변화량과 CES-D 우울감도 양의 상관관계를 보였다. KISMI 사회적위축 변화량과 CES-D 우울감 변화량은 양의 상관 관계를 보였다. 그 외의 지표들은 삶의 질 변화량과 상관관계를 보이지 않았다. 결 론 : 치료를 통해 우울감의 변화가 커지면 삶의 질 변화가 커지지만 병식, 낙인 및 조현병 증상의 변화는 삶의 질의 변화를 가져오지 않는다.
Objectives : This study investigated the association between the changes in quality of life, stigma, insight, depression and the symptoms of schizophrenia over time in schizophrenia patients. Methods : The demographic characteristics of 30 Schizophrenia patients who diagnosed by DSM-5 interviews were collected from January 2015 to January 2016. To evaluate the quality of life, stigma, insight, depression, and and the symptoms of schizophrenia, SQLS-R4K, KISMI, SUMD-K, Birchwood insight scale, CES-D, and PANSS were checked twice at 4-week intervals. Each scale was calculated on the change of total score and the change of sub-domains score to evaluate the correlation with the change in quality of life. And baseline and 4 weeks later for each scale was analyzed paired t test and each change was analyzed by the Pearson correlation test. Results : The change in Quality of Life through SQLS-R4K was positively correlated with the change in depression through CES-D. The change in social withdrawal through KISMI was positively correlated with the change in depression thorough CES-D. The other scales were not correlated with the change in Quality of Life. Conclusion : The change in Quality of Life score over time is associated, independently of changes in the insight, stigma and the symptoms of schizophrenia, with declining depression.
