Comparative Effectiveness of Korean Medicine Treatment for Children with Idiopathic Short Stature : A Systematic Review and Network Meta-Analysis

특발성 저신장은 최종 성인 키와 아동의 심리사회적 측면에 영향을 미친다. 한약과 침술 등을 포함한 한의치료는 오랜 기간 동안 특발성 저신장 치료에 널리 사용되어 왔다. 이 연구의 목적은 체계적 문헌고찰 및 네트워크 메타분석를 통해 특발성 저신장에 대한 한의치료의 비교 효과를 확인하는 것이었다. 특발성 저신장 소아에 대한 다양한 한의치료를 평가한 무작위배정 비교 임상시험을 검색하기 위해 14개의 전자 데이터베이스에서 포괄적인 검색을 수행하였다. 1차 결과지표는 성장 속도였다. 네트워크 메타분석을 수행하기 위해 Stata 버전 16.1이 사용되었다. 네트워크 메타분석 효과 추정치, 근거수준 및 치료 순위를 기반으로, GRADE 부분적 맥락화된 프레임워크에 따라 네트워크 메타분석 기반 결론을 도출하였다. 21편의 무작위배정 비교임상시험이 포함되었다. 모든 중재들을 고려했을 때 연간 성장 속도에 대해 한약과 성장호르몬 병행 치료는 아마도 중등도의 유익한 효과가 있다. 혈위첩부요법과 지압 병행치료, 성장호르몬, 한약과 침술병행치료, 혈위첩부요법, 한약과 영양 보충제 병행치료, 침술 및 한약은 아마도 약간의 유익한 효과가 있다. 또한, 지압은 약간의 유익한 효과가 있을 수 있다. 6개월간 성장 속도의 경우, 한약과 성장호르몬병행치료 및 성장호르몬 단독치료는 아마도 큰 유익한 효과가 있다. 한약과 지압 병행치료는 아마도 중등도의 유익한 효과가 있으며, 한약, 한약과 추나 병행치료 및 영양 보충제는 약간의 유익한 효과가 있을 수 있다. 모든 한의치료 중재는 치료 6개월 후 뼈 나이 변화에 미미한 영향을 미칠 수 있다. 한약과 성장호르몬 병행 치료는 성장호르몬 또는 한의치료 단독에 비해 성장 속도에 최적의 효과를 나타낼 수 있다. 일반적으로 한의복합치료가 단독치료보다 더 효과적인 경향을 보였다. 향후 잘 설계된 전향적 장기 대조군 비교 임상연구를 바탕으로 재확인되어야 한다.

Idiopathic short stature (ISS) can affect the final adult height and the psychosocial well-being of children. Korean medicine, including herbal medicine (HM) and acupuncture, has been widely used for the treatment of ISS for many years. This study aimed to determine the comparative effectiveness of Korean medicine treatment for ISS using a systematic review and network meta-analysis (NMA). A comprehensive literature search was performed using 14 electronic databases. Randomized controlled clinical trials (RCTs) that evaluated various Korean medicine treatment strategies for children with ISS were selected. The primary outcome was growth velocity (GV). The Stata version 16.1 was used to perform the NMA. Based on the NMA effect estimates, the quality of evidence, and treatment ranking, the NMA-based conclusions were drawn according to the GRADE partially contextualized framework. Twenty-one RCTs were included. When considering all the interventions, HM plus recombinant human growth hormone (GH) probably has a moderate beneficial effect for the GV per year. Acupoint herbal patching (AHP) plus acupressure, GH, HM plus acupuncture, AHP, HM plus nutritional supplements, acupuncture, and HM probably have a small beneficial effect. In addition, acupressure might have a small beneficial effect. For GV per 6 months, HM plus GH and GH alone probably have a large beneficial effect. HM plus acupressure probably has a moderate beneficial effect, while HM, HM plus Chuna, and nutritional supplements might have a small beneficial effect. All Korean medicine treatment interventions might have a trivial effect on the change in bone age change 6 months post-treatment. HM plus GH showed an optimal effect on GV when compared with GH or Korean medicine treatment alone. Generally, Korean medicine combination treatment tended to be more effective than Korean medicine treatment monotherapy. Well-designed prospective longitudinal controlled clinical studies are warranted in the future.
