임상사례기반 SBAR를 적용한 급성악화환자간호 시뮬레이션 교육이 의사소통명확성, 의사소통자신감, 임상추론역량에 미치는 효과

Effects of Simulation Education for Acute Deteriorating Patient Care using Clinical Case-based SBAR on Communication Clarity, Communication Confidence, and Clinical Reasoning Competency of Nursing Students

목적 본 연구는 임상사례기반 SBAR를 적용한 급성악화환자간호 시뮬레이션 교육이 의사소통명확성, 의사소통자신감, 임상추론역량에 미치는 효과를 확인하고자 수행되었다. 방법 연구설계는 단일군 사전사후에 의한 유사실험 연구로, 연구 대상은 G시 소재 일개 간호학과 4학년 학생 총 41명이었다. 자료수집을 위한 설문조사는 교육전, 후 2차례 진행하였다. 수집된 자료는 SPSS 23.0 프로그램을 이용하여 분석하였다. 대상자의 일반적 특성은 빈도와 백분율, 평균과 표준편차 등을 사용하여 분석하였으며, 교육 전 후 의사소통명확성, 의사소통자신감, 임상추론역량의 차이는 paired t-test로 분석하였다. 결과 임상사례기반 SBAR를 적용한 급성악화환자간호 시뮬레이션 교육 후가 교육 전에 비해 의사소통명확성(t=7.906, p<.001), 의사소통자신감(t=24.263, p<.001) 및 임상추론역량(t= 6.427, p<.001)이 유의하게 높은 것으로 나타났다. 결론 본 연구는 국내에서 처음으로 시도된 급성악화환자 간호 시뮬레이션 교육의 효과평가 연구로, 본 연구결과를 통해 향후 국내 급성악화환자 간호 교육 과정을 운영하는데 기초 자료로 활용할 수 있을 것이다. 또한 본 연구결과를 토대로 다양한 컨텐츠 및 교수법을 적용한 급성악화환자간호 시뮬레이션 교육을 확대하여 간호학생의 급성악화환자 간호에 대한 역량을 향상시킨다면 궁극적으로 급성악화환자의 예후향상에 기여할 것이다.

Objectives The purposes of this study was to identify the effect of nursing simulation education for acute deteriorating patients using clinical case-based SBAR on communication clarity, communication confidence, and clinical reasoning competency. Methods This quasi-experimental study used a one group pre-post design. The participants were 41 nursing students. A survey to collect data was conducted twice, before and after education. Data were analyzed using the SPSS 23.0 program with descriptive statistics and paired t-test. Results Communication clarity (t=7.906, p<.001), communication confidence (t=24.623, p<.001) and clinical reasoning competency (t=6.427, p<.001) were significantly improved after education compared to before. Conclusions This study is the first study to evaluate the effectiveness of nursing simulation education for acute deteriorating patients conducted in Korea. The results of this study can be used as basic data to develop a nursing education curriculum for patients with deterioration in Korea in the future. In addition, based on the results of this study, if nursing students' competency in nursing patients with acute deterioration is improved by expanding nursing simulation education by applying various contents and teaching methods, it will ultimately contribute to improving the prognosis of patients with acute deterioration.
