자폐스펙트럼장애 아동에서 통합적 조기중재치료가 언어 발달에 미치는 효과에 대한 고찰

A Study on the Effect of Integrative Early Intervention Therapy on Language Development in Children with Autism Spectrum Disorder

목적 본 연구는 자폐스펙트럼장애 아동에게 언어치료, 응용행동분석, 감각통합치료를 종합적으로 시행한 후 치료 효과를 평가해 봄으로써 통합적 조기 중재의 치료효과에 대한 정보를 제공하기 위해 고안되었다. 방법 P 대학병원 발달장애 행동발달증진센터에서 6개월 이상 통합적 중재 치료를 받은 30명의 아동들을 대상으로 6개월간의 통합적 중재 치료 후 치료 전과 치료 후의 월평균 언어성장속도를 비교하였고, 그 중 1년 이상 치료받은 16명의 아동들에게서는 첫 6개월과 6개월에서 1년간의 월평균 언어성장속도를 비교하였다. ADHD의 유무에 따라 아동의 언어성장속도의 차이를 비교하였고, 치료 후 언어성장속도를 예측할 수 있는 인자를 찾기 위해 ADI-R, CARS2, GARS, ABC, SCQ과 치료후의 언어성장속도와의 연관성을 확인하였다. 결과 6개월 이상 치료받은 30명의 치료 후 첫 6개월간의 월평균 언어성장속도는 수용언어와 표현언어 모두 유의하게 상승한 반면, 1년간 치료를 받은 16명의 치료 후 6개월에서 1년간의 월평균성장속도는 치료 후 첫 6개월간의 언어성장속도와 수용언어와 표현언어 모두 유의한 차이를 나타내지 않았다. ADHD를 가진 ASD 아동과 ADHD가 없는 ASD 아동간의 월평균 언어성장속도는 유의한 차이를 나타내지 않았지만, ADHD를 가진 군이 수용언어와 표현언어 모두 낮게 나타났다. 치료 6개월 후의 월평균 언어성장률은 수용언어, 표현언어 모두 ADI-R A, SCQ와 유의한 부적 상관관게를 나타낸 반면, 6개월에서 12개월의 월평균 언어성장률은 수용언어가 ADI-R B, 표현언어는 ADI-R A, B, CARS2와 유의한 부적 상관관계를 나타냈다. 결론 본 연구는 응용행동분석, 감각통합치료, 언어치료를 동시에 실시한 통합적 조기중재치료가 아동의 언어성장에 효과가 있다는 점을 확인함으로써 통합적 조기중재치료의 근거기반을 확립했다는 점에서 그 의의가 있다고 할 수 있다.

Objectives The purpose of this study is to provide information on the therapeutic effect of integrated early inter-vention by comprehensively performing speech therapy, applied behavior analysis, and sensory integration ther-apy in children with autism spectrum disorder. Methods 30 children treated for more than 6 months at the P University Hospital's Autism and developmental dis-order treatment center were evaluated at the mean monthly language growth rate after 6 months of integrated intervention. Results At the start of treatment, the mean monthly growth rate of the receptive languages was 0.35 months (SD=.22, S.E. Mean=.04), and the mean monthly growth rate of the expressive languages was 0.33 months (SD=.20, S.E. Mean=.04), whereas the mean monthly growth rate of the receptive languages after 6 months’ treatment was 1.31 months (SD=1.61, S.E. Mean=.29), and the mean monthly growth rate of the expressive lan-guages was 1.13 months (SD=1.31, S.E. Mean=.24) and paired t-tests showed significant differences. Conclusions This study is significant in that it established the evidence base for integrated early intervention ther-apy by confirming that integrated early intervention therapy is effective for children's language growth.
