혈액투석 환자의 신체상, 증상 경험이 삶의 질에 미치는 영향

Effects of Body Image and Symptom Experience on Quality of Life in Patients with Hemodialysis

본 연구는 혈액투석 환자의 신체상, 증상 경험, 삶의 질을 파악하고 삶의 질에 영향을 미치는 요인 및 관계를 확인하고자 한 서술적 조사연구이다. 자료수집은 2023년 7월 20일부터 2023년 8월 11일까지이며, J도 소재 대학병원에서 정기적인 혈액투석 치료를 받는 말기신부전 환자 153명을 대상으로 하였다. 자료 분석은 SPSS/WIN 28.0 프로그램을 이용하여 기술통계, One way-ANOVA, Pearson’s correlation coefficients와 다중회귀분석을 실시하였다. 대상자의 신체상, 증상 경험, 삶의 질 간의 상관분석 결과, 삶의 질은 증상 경험(r=-.31, p<.001)과 유의한 음의 상관관계가 있었으며, 신체상(r=.61, p<.001)과 유의한 양의 상관관계가 있었다. 신체상과 증상 경험(r=-.18, p=.023)은 유의한 음의 상관관계가 있었다. 대상자의 삶의 질에 영향을 미치는 요인은 신체상(β=.46, p<.001), 가구 월 평균수입(300만원 이상)(β=.22, p=.002), 증상 경험(β=-.20, p=.001) 순이었으며, 이 모형의 설명력은 47%로 나타났다(F=46.78, p<.001). 본 연구결과를 토대로 혈액투석 환자의 삶의 질 향상을 위해서는 신체상, 증상 경험을 정확하게 사정하고 중재할 필요성이 있으며, 증상 경험을 줄이고 신체상을 증진시키는 교육 및 통합 관리 프로그램 개발과 이를 위한 혈액투석실 간호사의 업무 표준 지침 확립이 요구된다.

The purpose of this study is to determine the effect of hemodialysis patients' body image and symptom experience on their quality of life. The data was targeted at patients who were diagnosed with end-stage renal failure at a tertiary general hospital located in J province and receiving regular hemodialysis treatment. The data collection period was from July 20, 2023 to August 11, 2023, and 160 structured questionnaires were distributed, of which 153 were used in the final analysis. Data were analyzed using t-test, One-way ANOVA, Pearson correlation coefficients and multiple regression analysis with the SPSS 28.0 program. Through multiple regression analysis, the factors influencing quality of life were body image (β=.46, p<.001), having an average monthly household income (over 3 million won)(β=.22, p=.002), and symptom experience (β=-.20, p=.001). The combined explanatory power for quality of life was determined to be 47%. It is necessary to accurately assess and intervene, develop educational programs and integrated management programs that reduce symptom experience and improve body image, and establish work standard guidelines.
