기능의학 증례 보고: 롱코비드와 부신 피로 동반 환자 1례

‘코로나 후유증, 또는 롱코비드(long COVID)’는 급성코로나19 감염증(COVID-19)에서 회복된 이후에도 상당기간 지속되는 다양한 증상과 합병증을 뜻한다. 대표적인증상들로 피로감, 두통, 미각 및 후각 이상, 만성 기침,호흡곤란, 집중력 및 기억력 저하(brain fog), 수면 장애,우울 및 불안, 탈모 등이 있다. 롱코비드의 발생률은 연구결과에 따라 차이가 있지만 코로나19 확진자의 20-60% 정도로 보고되고 있다. 하지만 이러한 높은 발생률과 의학적, 사회적 부담에도 불구하고 아직 롱코비드에 대한 명확한 정의나 객관적인 진단 기준이 없고, 의학적인 검사 및 치료도 연구나 근거가 부족한 상황으로 진료에 어려움이 있다. 하지만 기능의학적으로, 특히 부신 기능 저하와 회복, 부신 피로(adrenal fatigue)의 관점에서 롱코비드를 접근하여 관련 혈액검사 및 소변 유기산 검사 등을 시행하고, 적극적인 생활 습관 교정 및 경구, 주사 영양치료를 활용한다면 환자에게 추가적인 도움을 줄 수 있을 것이다. 본 원고에서는 롱코비드 환자에서 부신 피로가 확인되어 기능의학적으로 치료 후 호전된 사례를 통해 롱코비드와 부신의 관계 및 기능의학적 접근법에 대해 고찰해 보고자 한다.

Long COVID, also known as post-acute sequelae of SARS-CoV-2 infection or post-acute COVID-19 syndrome, refers to a range of symptoms and complications that persist more than four weeks after diagnosis. Typical symptoms include fatigue, headache, anosmia, ageusia, chronic cough, shortness of breath, impaired concentration and memory (brain fog), sleep disturbances, depression, anxiety, and hair loss. The incidence of long COVID varies according to the data, but it has been reported in approximately 20-60% of acute COVID-19 patients. However, despite its high incidence and significant medical and societal burden, there is currently no clear definition or objective diagnostic criteria for long COVID. Its diagnosis and treatment options are also limited and not established due to insufficient evidence. Nevertheless, a functional medicine approach, especially focused on adrenal functional and adrenal fatigue, can help long COVID patients. Relevant blood tests and urinary organic acid profile can be utilized, and lifestyle modification, oral or intravenous nutrition therapies can be implemented. In this article, we report a clinical case of long COVID with adrenal fatigue, demonstrating significant improvement after functional medical approach and management.
