임상간호사의 환자안전간호활동에 영향을 미치는 요인

Factors influencing ptient safety nursing activities of clinical nurses

본 연구는 임상간호사의 환자안전간호활동, 공유리더십, 근거기반실무역량의 정도를 파악하고 환자안전간호활동의 영향요인을 확인하여 환자안전간호활동 증진방안을 위한 기초자료를 제공하고자 시행되었다. 본 연구는 서술적 조사연구로 D시 일개 종합병원에서 근무하는 158명 임상간호사를 대상으로 설문조사하였다. 수집된 자료는 IBM SPSS 28.0프로그램을 이용하여 Mann-Whitney U test, Kruskal-Wallis H test와 위계적 다중회귀분석을 시행하였다. 환자안전간호활동과 공유리더십은 각각 5점 만점에 4.77±0.33점, 3.98±0.45점이었으며, 근거기반실무역량은 7점 만점에 4.58±0.60점이었다. 최종 회귀분석결과 공유리더십(β=.28, p=.001)이 높을수록, 소속부서의 구성 인원수 중 40명 미만(β=.17, p=.018)인 경우 환자안전간호활동에 유의한 영향을 미쳤다. 변수들의 환자안전활동의 설명력은 17.2%였다. 본 연구결과를 통해 높은 공유리더십과 40명 이하의 소속부서의 구성인원 수가 환자안전간호활동에 중요한 영향요인임을 확인하였다. 따라서 적절한 인력의 배치와 임상간호사의 공유리더십을 활성화하기 위한 간호관리전략방안과 교육프로그램 개발이 필요하겠다.

Purpose: The purpose of this study was to identify the degree of patient safety nursing activities, shared leadership, and evidence-based practice competency of clinical nurses, and to explain factors influencing patient safety nursing activities. Methods: In this descriptive survey study, the subjects of this study were 158 clinical nurses working in a general hospital in D city, South Korea. Data were collected using structured self-reporting questionnaires and analyzed with descriptive statistics, Mann-Whitney U test, Kruskal-Wallis H test, and hierarchical multiple regression analysis using the IBM SPSS 28.0 program. Results: Patient safety nursing activities and shared leadership scored 4.77±0.33 and 3.98±0.45 out of 5 points, respectively, while evidence-based practice competence scored 4.58±0.60 out of 7 points. Factors influencing patient safety nursing activities in clinical nurses were shared leadership(β=.28, p=.001) and the number of staff at the current department(β=.28, p=.001). The final model explained 17.2% of variance of clinical nurses’ patient safety nursing activities. Conclusion: The results suggest the advanced management strategies for shared leadership and nursing staffing need to enhance patient safety nursing activities. Moreover, it is necessary to develop nursing education programs improve shared leadership among clinical nurses.
