The purpose of this study is to analyze the cost-effectiveness of health care-based care for the elderly vs. care for the elderly that is not based on health care. For this research, family members who are primary caregivers that utilize health care-based care (i.e., a long-term care hospital) or non-health care based care (i.e., a nursing home for the elderly) for the elderly who had qualified to receive long-term care services by the National Health Insurance Corporation were selected and surveyed. Among the collected data, data for 40 primary caregivers utilizing health care-based care and 48 primary caregivers utilizing non-health care-based care were used for this study. Cost-effectiveness analyses were conducted by utilizing the incremental cost-effectiveness ratio. The results of this study can be summarized as follows. First, the primary stressors of caregiving have cost-effectiveness in health care-based care relative to non-health care-based care. That is, role overload, role captivity, and loss of intimacy are cost-effective. Second, the secondary stressors of caregiving as role strains are different on cost-effectiveness. That is, among the secondary stressors, work conflict has only cost-effectiveness but family conflict and loss of self have no cost-effectiveness. Third, depression as a caregiving stress has no cost-effectiveness.
