커뮤니티케어를 위한 방문간호 기관의 ICT 영상 협진 경험에 관한 연구: 건강관리강화 시범사업 참여기관을 중심으로

The Experience of ICT Video Collaboration in Visiting Nursing Centers for Community Care: Participating Organizations in the Health Care-Strengthening Pilot Project

본 연구는 초고령사회의 초입에 있는 한국 상황에서 커뮤니티케어를 위한 장기요양 재가 수급 노인의 건강관리에 있어 디지털(Digital)과 ICT(Information Communication Technology)를 활용한 영상 협진에 관한 경험을 탐색하였다. 연구대상자는 건강관리 강화 시범사업에 참여한 경험이 있는 방문간호 기관 실무자로 방문간호사 11명과 사회복지사 2명이 연구에 참여하였다. 수집된 자료는 Elo와 Kyngäs가 제안한 귀납적 내용분석(inductive content analysis) 방법으로 분석하였다. 분석 결과로는 ‘의사와 간호사가 영상 협진하며 건강관리를 제공함’, ‘참여자들은 만족스러운 반응을 보임’, ‘간호사의 일은 많아지고 의사들의 참여는 저조함’, ‘공단의 준비가 미흡함’, ‘현장 필요에 따른 제도 개선이 필요함’이라는 5개의 주제와 이에 따른 11개의 속성으로 범주화하였다. 연구 결과에 근거한 제언으로 커뮤니티케어를 위해 대면 진료가 어려운 재가 환자의 대면 진료 체계 불편을 해결하는 ICT 영상 협진이 본 사업화되는 것이 필요하며 이를 위해 사업 주체인 건강보험 공단이 의사들에 대한 홍보와 모집에 좀 더 적극적으로 관여할 필요가 있다. 또한 장기요양 재가 의료 서비스 담당 기관인 방문간호가 활성화되어야 하며 이를 위해 간호 수가에 대한 개선이 필요하다. ICT를 활용한 커뮤니티케어가 확장되기 위해서는 IoT(Internet of Things) 센서를 재가 돌봄에 활용하여 응급상황에 대처할 수 있는 보건·복지 서비스를 확대해 나가야 할 것이다.

This study explored the experience of video collaboration using digital and information communication technology (ICT) in the health management of the elderly receiving long-term care at home for community care in Korea at the early stages of a super-aging society. The subjects of the study were practitioners from visiting nursing organizations who had participated in pilot projects to strengthen health care. Eleven visiting nurses and two social workers participated in this study. The collected data were analyzed using the inductive content analysis method proposed by Elo and Kyngäs. Results include “Doctors and nurses provide health care through video collaboration,” “Participants are satisfied with the service,” “Nurses’ work increases, doctors’ participation is low,” “The corporation’s preparation is insufficient,” “Systems must be improved according to on-site needs.” The results were categorized into five themes and eleven attributes accordingly. The results indicate that ICT video collaboration must be implemented to overcome the inconvenience of the face-to-face treatment system for home-based patients who have difficulty receiving in-person doctors’s treatment. To this end, the National Health Insurance Corporation must be more actively involved as the project operator, which should promote and recruit doctors. In addition, nursing fees must be increased to motivate visiting nurses who provide long-term care home medical services. Expanding community care using ICT requires expanding emergency health and welfare services to incorporate internet-of-things (IoT) sensors for home care.
