언어행동기반의 조기집중행동중재(EIBI)가 자폐스펙트럼장애(ASD) 아동의 발달에 미치는 영향

The Effect of Early Intensive Behavioral Intervention(EIBI) Based on Verbal Behavior Analysis on the Development of Children with Autism Spectrum Disorder(ASD)

이 연구는 자폐스펙트럼장애(ASD) 아동에게 제공된 언어행동기반의 조기집중행동중재(EIBI)의 효과를 분석하였다. 본 중재가 ASD 아동의 전반적인 발달에 미치는 영향을 확인하고, 그것을 예측하는 변수를 파악하는 데 목적이 있다. 연구 대상은 ASD 진단을 받은 46명의 아동으로, 이들은 12개월간 행동분석(Applied Behavior Analysis, ABA) 전문 기관에서 언어행동분석을 기반으로 한 EIBI 중재를 받았다. 중재 전후로 ADOS-2 검사, 언어검사, 교육진단검사, 바인랜드 적응행동 척도, ELCAR 커리큘럼 평가를 통해 아동의 발달 수준을 평가하였다. 자료 분석 방법으로는 대응표본 t-검정과 중다회귀분석을 사용하였으며, 그 결과 EIBI 중재 후 ASD 아동의 자폐증상심각도, 언어발달, 발달연령, 적응행동, ELCAR 커리큘럼 점수에서 유의미한 향상이 나타났다. 또한, 발달연령의 향상을 예측하는 변수로는 자폐증상심각도가, ELCAR 커리큘럼의 성취도를 예측하는 변수로는 발달연령이 확인되었다. 발달연령의 세부 평가 항목에서는 모방이 중요한 예측변수로 나타났다. 이러한 결과를 바탕으로 국내 ASD 아동의 EIBI 중재의 효과와 한계점을 논의하였다.

This study examines the effects of Early Intensive Behavioral Intervention(EIBI) on the overall development of children with Autism Spectrum Disorder(ASD) and identifies predictive factors related to developmental improvements. The study involved 46 children diagnosed with ASD who underwent EIBI based on applied behavior analysis at a specialized institution for 12 months. Assessments of the children’s developmental levels were conducted before and after the intervention using the Ados-2 test, language assessments, educational diagnostic tests, the Vineland Adaptive Behavior Scales, and the Early Learner Curriculum and Achievement Record(ELCAR) curriculum evaluation. Data analysis was performed using paired sample t-tests and multiple regression analysis. The results show significant improvements in ASD symptom severity, language development, developmental age, adaptive behavior, and ELCAR curriculum scores following the EIBI intervention. Furthermore, ASD symptom severity was a significant predictor of improvement in developmental age, and developmental age was a predictor of ELCAR curriculum achievement. Within the developmental age subdomains, imitation emerged as an important predictor. These findings show the effectiveness and limitations of EIBI interventions for children with ASD in South Korea.
