Rethinking Health to Explore Directions for Care Practices in Healthcare

본 연구는 현대 의료 체계에서 돌봄이 부차적 활동으로 축소되거나 생의학적 접근에 의해 배제되는 구조적 문제를 비판적으로 검토하고, 돌봄 실천을 재구성하기 위한 건강 개념의 방향성을 모색한다. 생의학적 모델은 질병을 생물학적 현상으로 제한하여 치료의 체계화와 과학적 정교화를 이루었으나, 환자의 정서적·사회적 요구를 포함한 건강의 맥락적 복잡성을 반영하지 못하는 한계를 드러낸다. 이러한 한계는 돌봄을 기술 중심적이고 편중된 실천으로 축소시키며, 환자가 분절된 돌봄 경험을 겪게 하는 구조적 문제로 이어진다. 이에 다양한 건강 모델 및 돌봄 이론을 검토하여 돌봄 실천에 적합한 대안적 건강 개념을 탐구하였다. 본 연구는 건강 개념을 고정된 상태가 아닌 동적이고 맥락적 과정으로 이해해야 한다고 제안하며, 이를 통해 돌봄 실천이 환자의 변화하는 요구를 반영하고 지속적으로 조정되는 과정임을 강조한다. 보건의료에서의 돌봄 실천의 핵심은 환자와 의료진 간 협력적 공동 지식의 생성, 다학제적 협력, 자율성의 관계적 이해, 그리고 보건의료 내부와 외부의 다양한 행위자 간의 관계망 형성에 있다. 이러한 논의는 돌봄을 의료 체계의 주요 가치로 재정립하고, 변화하는 보건의료 환경에서 돌봄이 지속 가능하고 실질적인 실천으로 자리 잡는 데 기여할 것이다.

This study critically examines the structural issues in modern healthcare systems, where care is often sidelined or overlooked due to the dominance of biomedical approaches. It aims to redefine the concept of health as a foundation for reconstructing care practices. While the biomedical model has contributed to the systematization and scientific refinement of treatment by focusing on disease as a biological phenomenon, it has notable limitations in accounting the contextual complexity of health, including patients’ emotional and social needs. As a result, care is reduced to a fragmented, technology-driven practice, leading to disjointed patient experiences. To address these challenges, this study explores alternative health concepts and care theories to develop models better suited to comprehensive care practices. It proposes viewing health not as a fixed state but as a dynamic and contextual process—one that requires care practices to continuously adapt to and reflect patients’ evolving needs. At its core, care practice in healthcare involves fostering collaborative knowledge creation between patients and healthcare professionals, promoting multidisciplinary collaboration, adopting a relational understanding of autonomy, and importantly, building networks among diverse actors within and beyond healthcare systems. These discussions aim to reestablish care as a central value in healthcare and to make it both sustainable and practical within the constantly evolving healthcare environment.
