Long-term Effects of Pulmonary Rehabilitation and Self-Exercise on the Physical Function of an Older Adult with Small Cell Lung Cancer: A 5-Year Case Study of a 79-Year-Old

Purpose: This case report aimed to share the long-term follow-up results of an older patient with small cell lung cancer (SCLC), focusing on the challenges and benefits of pulmonary rehabilitation (PR). Methods: A 79-year-old patient with limited-stage SCLC and chronic obstructive pulmonary disease (COPD) received chemoradiotherapy and participated in 15 PR sessions over 2 months, followed by self-exercise and management education, including inspiratory muscle training (IMT), during outpatient visits over 38 months. Physical function was assessed eight times during follow-up. Results: The initial evaluation revealed forced vital capacity at 45.9%, forced expiratory volume in 1 s at 43.3%, maximal inspiratory pressure of 30 cmH2O, maximal expiratory pressure of 84 cmH2O, and a 6-min walk test (6MWT) distance of 497 m. After the acute exacerbation of COPD and SCLC symptoms, the patient’s respiratory muscle strength and body composition improved, and the 6MWT distance was well maintained above 350 m. Conclusion: This case demonstrates that even IMT and self-management education in an outpatient setting can help maintain long-term physical function in older patients with SCLC. PR programs are crucial for improving the quality of life and physical function of these patients.
