Adrenal insufficiency is a rare complication of adrenal metastasis and can be the first clinical manifestation of an underlying malignancy. Herein we report the case of a 34-years-old male, with a history of heavy smoking, who presented to the emergency department after experiencing several weeks of weakness, rapid weight loss, skin pigmentation. The biological workup performed in emergency was in favor of adrenal insufficiency. The etiological investigation revealed a large lung mass which, after biopsy, turned out to be a lung adenocarcinoma. The lung cancer subsequently caused adrenal metastasis and therefore adrenal insufficiency. This case highlights the importance of considering adrenal metastasis as a possible cause of adrenal insufficiency in patients with a history of malignancy.
