백내장 환자에서 우연히 발견된 토피도 황반병증

Torpedo Maculopathy in a Cataract Patient

목적: 토피도 황반병증(torpedo maculopathy, TM) 1예를 보고하고자 한다. 증례요약: 73세 여자가 우안 시력저하로 백내장 수술을 위해 내원하였다. 우안 나안시력은 0.3이었으며, 중등도 핵백내장 및 후낭하혼탁이 있었다. 안저검사상 우안 중심와 이측 및 상비측에 어뢰모양 저색소 경계의 과색소 병변이 관찰되었다. 수술 후 7일째 최대교정시력은 1.0로 회복되었다. 빛간섭단층촬영에서 병변의 외망막층 결손과 국소맥락막함몰이 관찰되었으며, 빛간섭단층촬영혈관조영술에서 맥락막모세혈관층의 결손이 있었다. 안저자가형광에서 과형광경계의 저형광병변이 보였다. 인도시아닌그린검사에서 저형광이관찰되었으며, 시야검사상 국소 암점이 있었다. 결론: 저자들은 우연히 진단된 TM 1예를 보고하고자 하였으며, 특히 국소맥락막함몰이 동반된 경우로, 맥락막신생혈관의 발생 위험성및 시야결손이 진행될 수 있어 추적관찰이 필요하다.

Purpose: We present a case of torpedo maculopathy (TM) found incidentally after cataract surgery. Case summary: A 73-year-old female presented for cataract surgery in her right eye. Her preoperative uncorrected visual acuity was 0.3. The patient had a moderate nuclear cataract with posterior subcapsular opacity. Fundus examination revealed a torpedo- shaped hyperpigmented lesion surrounded by a hypopigmented margin, located at the superonasal retina and temporal side of the fovea. Seven days after cataract surgery, her best corrected visual acuity was 1.0. Post-operative optical coherence tomography revealed an outer retinal defect with focal choroidal excavation. Optical coherence tomography angiography demonstrated a deficit in the choroidal capillary layer. Fundus autofluorescence showed hypo-fluorescent lesions within the hyper-fluorescent margins. Hypocyanescence in the choroidal arterial phase was seen on indocyanine green angiography. Localized microscotomas were identified on the visual field exam. Conclusions: TM, associated with localized choroidal excavation, was detected incidentally in a patient scheduled for cataract surgery. Due to the potential risk of choroidal neovascularization and the progression of visual field defects in TM, long-term follow- up is needed.
