Both Han and Tibetan medicine are rooted in ancient Chinese philosophy, profoundly influenced by the theory of Yin and Yang. Although their expressions and theoretical frameworks differ, their core ideas consistently emphasize the dynamic balance between the human body and nature. Historically, medical exchanges between the Han and Tibetan traditions were frequent. During the Tang Dynasty, Princess Wencheng’s journey to Tibet brought with her Chinese medical texts and practitioners, which facilitated the absorption and transformation of Han medical theories within Tibetan medicine. Existing studies mainly focus on single medical systems, while systematic comparative and interactive analyses of the Yin-Yang concepts in Han and Tibetan medicine remain limited. This paper, from the perspectives of medical history and philology, systematically traces the independent formation and development of the Yin-Yang theory in Han medicine and the hot-cold theory in Tibetan medicine. It further examines their collision and integration during historical exchanges such as the Tang-Tubo interactions, aiming to deepen the understanding of the formation of the Chinese national medical community and to provide a theoretical foundation for the integration and development of contemporary ethnic medicine.
