개 교상에 의한 위눈꺼풀 전층 결손 1예

A Case of Dog Bite-Induced Upper Eyelid Full-Thickness Defect

목적: 개에 의한 교상으로 눈물점과 눈물소관 손상을 포함한 위눈꺼풀의 전층 결손이 발생한 환자에서 위눈꺼풀 및 눈물소관 재건술을 시행한 1예를 경험하여 보고하고자 한다. 증례요약: 46세 여자 환자가 개에 물려 발생한 좌측 위눈꺼풀의 결손으로 응급실 통해 내원하였다. 좌안 위눈꺼풀에 위눈물점과 근위부 위눈물소관을 포함한 피부전층 결손이 있었다. 응급실 내원 당시 환자는 위눈꺼풀 피부전층과 속눈썹, 눈꺼풀 결막이 포함된 절편을 가지고 왔으며 절편을 재부착하여 위눈꺼풀 재건술 및 실리콘관을 이용한 위눈물소관 재건술을 시행하였다. 수술 3주 후 외래내원 시 위눈꺼풀 속눈썹 소실 및 피부의 상피괴사 소견이 보였고, 수술 2개월 후 위눈꺼풀 상피괴사 소견은 호전되었다. 눈물소관재건 시 삽입한 실리콘관은 수술 5개월 후 제거하였고 눈물 흐름 소견은 없었으나, 경도의 눈꺼풀처짐과 경도의 토안, 속눈썹 소실이관찰되었다. 결론: 개 교상으로 인한 위눈꺼풀 전층 결손 환자에서 위눈꺼풀 절편 이식 및 눈물소관 재건술을 통해 성공적인 생착과 재건이 가능하였으나, 이식한 위눈꺼풀 절편의 부분적인 상피 괴사로 토안 및 속눈썹소실이 남았다.

Purpose: We present a case of full-thickness upper eyelid and upper canalicular reconstruction following a dog bite that caused a full-thickness eyelid defect with associated punctal and canalicular injury. Case summary: A 46-year-old woman presented to the emergency room with a full-thickness defect of the left upper eyelid caused by a dog bite. The injury involved the proximal upper canaliculus and punctum. The patient brought excised tissue containing full-thickness skin, eyelashes, and conjunctiva. Reconstruction of the upper eyelid and upper canaliculus was performed under general anesthesia. At the 3-week follow-up, the patient exhibited eyelash loss and necrotic crusting on the left upper eyelid. At the 2-month follow-up, the necrotic crust had improved. After 5 months, the nasolacrimal silicone tube was removed. Although the patient reported no tearing, mild ptosis, lagophthalmos, and eyelash loss persisted. Conclusions: Full-thickness upper eyelid and upper canalicular reconstruction was successfully performed in this case of a dog bite-induced defect. However, mild ptosis, lagophthalmos, and eyelash loss persisted.
