목적 지방자치단체의 의료관광 조례 제정이 확산되고 있으나, 대부분 예산·인력·조직 등 집행수단이 미비하며, 지역의 의료자원이나 관광 인프라와의 연계도 미흡한 실정이다. 이에 본 연구는 전국 지자체의 의료관광 조례를 체계적으로 분석하여 정책 실효성을 저해하는 요인을 규명하고, 실질적으로 작동 가능한 제도 설계 방향을 제시하는 것을 목적으로 한다. 방법 이 연구는 전국 35개 의료관광 관련 조례를 대상으로 분석을 수행하였다. 조례의 명칭과 구성 등 기본사항을 고찰하고, 텍스트마이닝 기법 중 키워드 분석과 LDA토픽모델링으로 조례의 핵심 요소 및 주제를 도출하여 내용분석 단계의 분석틀로 활용하였다. 결과 키워드 분석 결과, 광역지자체는 산업육성 및 글로벌 시장 지향적 접근을 보이는 반면, 기초지자체는 의료인프라 강화와 거버넌스 기반의 접근 방식을 취하는 차별점이 확인되었다. 토픽모델링 결과, 제도적 토대와 정책 방향, 사업 실행과 자원 운용, 협력적 의사결정과 전달체계, 의료 한류의 글로벌 시장개척 등 4개 토픽이 도출되었으며, 이는 의료관광 정책이 다층적이고 복합적인 구조를 갖고 있음을 시사한다. 결론 연구결과를 바탕으로 지방자치단체 차원의 전담조직 구성, 광역과 기초지자체 간 유기적인 협력 증진, 지역연계 상품개발 지원, 지역 의료기관의 부담완화 및 역량강화 지원을 위한 조례 제·개정의 실천적 과제를 제안하였다.
Purpose Despite the proliferation of medical tourism ordinances by local governments, most lack essential implementation measures including budgets, personnel, and organizational frameworks, and fail to adequately connect with regional medical resources and tourism infrastructure. This study systematically analyzes these ordinances to identify effectiveness barriers and propose practical institutional design directions. Methods This study analyzed 35 local government medical tourism ordinances. After examining basic characteristics (titles, structures), we employed keyword analysis and LDA topic modeling to extract core themes, which formed the analytical framework for content analysis. Results Keyword analysis showed that metropolitan governments focus on industry development and global markets, while basic-level governments emphasize medical infrastructure and govern-ance. Topic modeling extracted four themes: institutional foundation, resource management, col-laborative systems, and global market development via medical Hallyu, indicating the multi-lay-ered complexity of medical tourism policies. Conclusion The study proposes practical ordinance revisions including: establishing dedicated organizations, strengthening metropolitan-basic level cooperation, supporting regional product development, and providing assistance to local medical institutions for burden reduction and ca-pacity building.
