모낭충 안검염에서 국소 이버멕틴 1% 연고의 효과

Efficacy of 1% Ivermectin Ointment for Demodex Blepharitis

목적: 이 연구는 모낭충(demodex)에 의한 만성 안검염을 가진 환자들에게 국소 이버멕틴 (ivermectin) 1% 연고 치료 전후의 충체수를 비교하여, 이버멕틴 1% 연고의 효과를 알아보고자 하였다. 또한, 눈꺼풀 인설 및 눈꺼풀 모세혈관확장증의 변화도 분석하고자하였다. 대상과 방법: 저자들은 모낭충 안검염이 의심되어 속눈썹을 뽑아 모낭충이 발견된 38명의 환자를 대상으로, 국소 1% 이버멕틴 연고를8주 동안 주 1회 도포하고 치료 전후 속눈썹 한 개당 충체 수 및 윗눈꺼풀의 눈꺼풀 비듬, 눈꺼풀 모세혈관확장증 단계를 비교 분석하였다. 결과: 총 38명의 환자를 분석하였으며, 평균 나이는 61.8 ± 10.7세로 15명은 남자, 23명은 여자였다. 국소 1% 이버멕틴 연고를 8주동안 주 1회 도포 후 속눈썹 한 개당 모낭충 수, 눈꺼풀 비듬 등급, 눈꺼풀 모세혈관확장증 등급이 통계적으로 유의미하게 감소하였다(각각, p<0.001, p<0.001, p=0.008). 치료 후 각막 상피 세포 결손 등의 합병증은 발견되지 않았다. 결론: 이버멕틴 1% 연고는 모낭충 안검염에서 모낭충 충체를 거의 100% 감소시키며, 눈꺼풀 비듬 및 모세혈관확장증도 유의하게 감소시켰다.

Purpose: We evaluated the efficacy of topical 1% ivermectin ointment in treating chronic Demodex blepharitis by comparing Demodex mite density before and after treatment. In addition, we assessed changes in eyelid cylindrical dandruff and telangiectasia. Methods: In total, 38 patients suspected of Demodex blepharitis were included. Following the epilation of eyelashes and confirmation of mites, topical 1% ivermectin ointment was applied once weekly for 8 weeks. We analyzed the change in mite density per eyelash and the severity scores of upper eyelid cylindrical dandruff and telangiectasia before and after treatment. Results: We included 38 patients (mean age, 61.8 ± 10.7 years; 15 men and 23 women) with suspected Demodex blepharitis. Topical 1% ivermectin ointment applied once weekly for 8 weeks led to a statistically significant reduction in the mite density per eyelash, severity of upper eyelid cylindrical dandruff, and upper eyelid telangiectasia (p < 0.001, p < 0.001, and p = 0.008, respectively). Notably, no complications such as corneal epithelial defects were observed. Conclusions: Topical 1% ivermectin ointment demonstrated exceptional efficacy, nearly eradicating Demodex infestations in patients with Demodex blepharitis. This resulted in significant improvements in upper eyelid cylindrical dandruff and telangiectasia.
