악어눈물증후군에서 보툴리눔 독소 A 주사 치료: 증례보고

Botulinum Toxin A Injection for the Treatment of Crocodile Tear Syndrome: A Case Series

목적: 안면신경 마비 후 발생한 악어눈물증후군 환자에서 보툴리눔 독소 A를 눈물샘에 주사하여 증상과 검사 지표가 호전된 2예를보고하고자 한다. 증례요약: 증례 1: 좌측 안면마비 병력이 있는 36세 남성은 식사 시 좌안 눈물 과다로 내원하였다. 외안부 및 눈물 배출 검사상 이상은없었고, 좌안의 기본 눈물 분비량은 9 mm, 껌을 씹을 때는 23 mm로 증가하여 악어눈물증후군으로 진단하였다. 보툴리눔 독소 A 주사 4주 후 저작운동 시 눈물 분비량은 13 mm로 감소하였고, Munk 점수도 3에서 1로 증상이 개선되었다. 증례 2: 우측 안면신경마비 병력이 있는 40세 남성은 식사 시 우안 눈물 과다를 호소하였다. 외안부 및 눈물 배출 검사상 이상은 없었으며, 우안의 기본눈물 분비량은 13 mm, 저작운동 시 29 mm로 증가하여 악어눈물증후군으로 진단하였다. 보툴리눔 독소 A 주사 4주 후 눈물 분비량은14 mm로 감소하였고, Munk 점수도 4에서 1로 증상이 개선되었다. 결론: 보툴리눔 독소 A의 눈물샘 국소 주사는 효과적인 증상 경감과 지표 개선을 보였다.

Purpose: We report two cases of crocodile tear syndrome following facial nerve palsy, in which both symptoms and objective findings improved after lacrimal gland injection of botulinum toxin A. Case summary: Case 1: A 36-year-old man with a history of left facial palsy reported excessive tearing in the left eye during meals. External ocular examination and lacrimal drainage tests were unremarkable. The baseline Schirmer test measured 9 mm, which increased to 23 mm during mastication, confirming the diagnosis of crocodile tear syndrome. Four weeks after botulinum toxin A injection into the lacrimal gland, the Schirmer test value during chewing decreased to 13 mm, and the Munk score improved from 3 to 1. Case 2: A 40-year-old man with a history of right facial nerve palsy presented with excessive tearing in the right eye during meals. External ocular examination and lacrimal drainage tests were normal. The baseline Schirmer test measured 13 mm, which increased to 29 mm during mastication, consistent with crocodile tear syndrome. Four weeks after botulinum toxin A injection into the lacrimal gland, the Schirmer value during chewing decreased to 14 mm, and the Munk score improved from 4 to 1. Conclusions: Lacrimal gland injection of botulinum toxin A effectively reduces symptoms and improves subjective and objective outcomes in crocodile tear syndrome.
