The Effects of Playing Surfaces on Injuries: A Systematic Review

Objectives This systematic review aims to establish the influence of playing surface on sports injuries. Methods Fifteen studies were analyzed for injury rates, severity, types, and environmental factors. The final research were selected through a search on the electronic database (Pubmed, Scopus, EBASCO) following PRISMA guidelines. Bias and quality assessment of the research were conducted using the Modified Downs and Black checklist. Results A total of 243 studies were identified, and 15 met the inclusion criteria for full analysis. Across these studies, no consistent gender-related differences in overall injury incidence were observed between playing surfaces. Injury incidence itself showed substantial inconsistency, with artificial turf and natural grass each demonstrating higher rates in different studies. Injury severity also produced conflicting findings, with some studies reporting higher se-verity on artificial turf and others on natural grass. In contrast, injury type and location showed the clearest pattern across studies: natural grass was more commonly associated with muscle strains, artificial turf with ligament sprains and joint-related injuries, and dirt fields with ankle and knee injuries. Environmental factors further contributed to variability, with several studies noting increased injury risk on artificial turf under dry and warm conditions. Conclusions Men frequently experienced hamstring and adductor tears, while women showed a higher incidence of anterior cruciate ligament (ACL) injuries. Artificial turf fields were associated with higher rates of ligament and joint injuries, whereas natural grass fields saw more muscle strains and tears. Additionally, environmental conditions in-fluenced injury risk: high temperatures increased injury risk on artificial turf, but in dry conditions without precip-itation, artifical turf provided a more stable playing environment, resulting in a lower injury risk compared to natural grass.
