Objectives Rapid weight loss (RWL) in weight-category sports often leads athletes to misuse synthetic diuretics, resulting in electrolyte imbalance, cardiovascular strain, and renal injury, while violating WADA regulations. This study aimed to evaluate the potential of Oryeong-san (五苓散) as a physiological, doping-safe alternative for fluid regulation in athletes. Methods A narrative review of clinical and pharmacological literature regarding Oryeong-san and synthetic diuretics was conducted using Google Scholar, RISS, KISS, and OASIS. Key terms included “Oryeong-san (五苓散),” “diuretics,” and “rapid weight loss.” Results Synthetic diuretics induce rapid diuresis but frequently cause dehydration, electrolyte imbalance, hypo-tension, arrhythmias, and renal injury. Oryeong-san (五苓散) demonstrated adaptive diuretic effects, supporting fluid and electrolyte homeostasis and showing renal- and cardiovascular-protective properties without concerns for dop-ing regulation. However, direct evidence in athletic RWL populations remains limited. Conclusions Oryeong-san (五苓散) may serve as a promising physiological and doping-safe adjunct to manage body fluid changes during RWL in athletes. Controlled sports-specific clinical studies are needed to determine optimal use, safety, and performance relevance.
