Objectives It aims to analyze running-related injury patterns from biomechanical and neuromuscular coordination perspectives and to propose a systematic, function-oriented rehabilitation strategy. Methods A structured narrative synthesis was conducted based on the seminar content and the clinical practice guideline framework. Running was conceptualized as a high-velocity weight-transfer task requiring coordinated multi-joint function. The analysis focused on (1) alignment and load-transfer relationships across the foot, knee, pel-vis, and trunk; (2) regional injury patterns; (3) sling system function and dysfunction; and (4) principles for a func-tion-based, staged rehabilitation approach. Results Running-related injuries were consistently associated with impaired energy transfer resulting from seg-mental malalignment (foot–knee–pelvis–trunk) and/or dysfunctional sling system coordination, leading to compen-satory movement patterns and localized tissue overload. Region-specific injury patterns were linked to identifiable kinetic-chain deficits and failures of neuromuscular control. Based on these findings, a systematic rehabilitation strategy was formulated, emphasizing integrated assessment of alignment and motor control, targeted restoration of sling function, and a progressive return-to-running guided by functional criteria. Conclusions Running should be approached as a complex coordinated movement rather than a simple aerobic exercise. Malalignment and sling system dysfunction may disrupt efficient weight transfer and increase injury risk. A function-oriented rehabilitation model integrating biomechanical alignment, neuromuscular coordination, and re-gion-specific injury mechanisms may support more effective injury prevention and return-to-running strategies.
