Objectives The paper aims to analyze the biomechanical role of hyperpronation (excessive inward rolling of the foot) in these injuries, review foot anatomy and normal gait, and propose a comprehensive conservative management protocol. Methods The approach involves analyzing the kinetic chain reaction initiated by excessive foot pronation, tracking its propagation superiorly from the foot to the knee, hip, and spine, and even up to the TMJ. The study summarizes clin-ical guidelines and reviews relevant biomechanical facts to establish a therapeutic protocol. Results Hyperpronation disrupts normal gait, initiating a kinetic chain that causes internal rotation of the tibia/femur and propagates pathologies to the knee (e.g., patellofemoral dysfunction) and spine (e.g., LBP). A comprehensive management protocol is proposed, emphasizing manual therapy, therapeutic exercise, and the strategic use of cus-tom foot orthoses to restore intrinsic foot muscle leverage and mitigate systemic effects. Conclusions Abnormal foot mechanics, particularly hyperpronation, are a significant source of systemic stress. The proposed integrated protocol is an essential strategy for the conservative management of running injuries by ad-dressing the underlying foot biomechanical dysfunction.
