웹 기반 통합기능돌봄시스템을 활용한 노인요양시설 완화돌봄의 다학제 협업 탐색

Exploring Interdisciplinary Collaboration for Palliative Care in Nursing Homes Using a Web-Based Integrated Function Care System

본 연구는 노인요양시설에 거주하는 복합장애 노인을 대상으로 의사소통 능력에 따른 다학제적 완화돌봄의 특성을 탐색한 후향적 질적 사례분석 연구이다. 연구팀이 개발한 웹 기반 통합기능돌봄시스템(Integrated Function Care, IFC system)을 활용하여 15명의 실무자(간호사 9명, 물리치료사 3명, 사회복지사 3명)가 32건의 사례기록을 제출하였다. 대상자는 모두 심한 인지 및 신체기능 저하 상태였으며, 의사소통 가능 여부는 돌봄의 우선문제와 중재 유형에 뚜렷한 차이를 보였다. 의사소통이 가능한 노인은 회상요법·가족상담 등 인지·심리사회 중재가 중심이 되었고, 불가능한 노인은 자세유지·욕창예방 등 신체적 안위 중심의 돌봄이 강조되었다. 이러한 결과는 복합장애 노인을 위한 완화돌봄에서 의사소통 수준별 맞춤중재와 직종 간 협업의 필요성을 보여준다. 본 연구는 IFC system을 활용한 디지털 기반 사례분석을 통해 요양시설 완화돌봄의 구조화와 다학제 협업모델 개발에 기초자료를 제공한다.

This study examined differences in interdisciplinary palliative care according to communication ability among older adults with complex disabilities in nursing homes. A retrospective qualitative case analysis was conducted using 32 case records submitted by 15 practitioners through the web-based Integrated Function Care (IFC) system. All residents had severe cognitive and physical impairment. Communication ability strongly shaped care patterns: communicative residents received more cognitive and psychosocial interventions, while non-communicative residents primarily received physical and comfort-focused care. These findings highlight communication level as a key factor in tailoring palliative care and emphasize the need for clear role division and collaboration among nurses, physical therapists, and social workers. The results provide a basis for developing communication-based assessment and interdisciplinary guidelines.
