Objectives The goal of this study is to establish a protocol for a systematic review for the effectiveness and safety of Chuna manual therapy for patients with shoulder im-pingement syndrome(SIS). Methods Following the PRISMA 2020 guidelines, seven electronic databases(MEDLINE/PubMed, the Cochrane Central Register of Controlled Trials, Embase, China National Knowledge Infrastructure, Korean studies Information Service System, Oriental Medicine Advanced Searching Integrated System. Research Information Service System) will be searched from their inception to the present. Randomized controlled trials(RCTs) that applied Chuna manual therapy to patients diagnosed with SIS will be included. The primary outcomes are pain intensity (VAS) and shoulder function scores (Constant-Murley Score, SPADI). Secondary outcomes include range of motion, quality of life, recurrence rate, and adverse events. Results Two independent reviewers will select studies, extract data, and assess the risk of bias using the Cochrane RoB 2.0 tool. When possible, meta-analysis will be conducted using Review Manager 5.4. Effect sizes will be expressed as standardized mean differences with 95% confidence intervals. Heterogeneity will be evaluated us-ing Higgins’ I² statistic; a fixed-effect model will be applied when heterogeneity is low, and a random-effects model when I² exceeds 50%. The certainty of evidence will be assessed using the GRADE approach. Conclusions This study will synthesize current evidence regarding the clinical ef-fectiveness and safety of Chuna manual therapy for SIS. The results will contribute to evidence-based decision-making in clinical practice and support the development of standardized guidelines for Korean manual medicine in musculoskeletal disorders.
