본 글은 어려운 환자 및 보호자를 규정하고 식별하는 수단으로 의료 현장에서 특수 기호와 알파벳 등의 표식을 사용하는 행위를 반성적으로 성찰하여 윤리적 간호 실천을 탐구하는 것이 목적이다. 윤리적 간호의 본질을 탐구하고 성찰하기 위해 본 글에서는 규범 윤리학으로부터 출발해 해석학적 윤리학으로 그 시선을 확장하였다. 간호사들이 특수 기호나 특정 알파벳 등을 통해 돌봄 대상자를 기호화하고 그 특성을 규정짓는 것은 인간 존재에 대한 편견 및 선입견을 초래하여 간호의 네 가지 메타 패러다임-사람, 건강, 환경, 간호-의 통합을 방해하고, 결과적으로 진정한 간호의 실천을 저해할 수 있다. 이에 따른 윤리적 논의를 위해 공리주의, 의무론, 덕 윤리 세 가지의 규범 윤리 이론을 먼저 살펴보겠지만, 규범 윤리학은 간호사들이 특수 기호를 기입하여 의사소통을 하게 되기까지 교류하던 돌봄 대상자의 경험과 맥락적 통찰력을 제공하는 데 한계가 있다. 따라서 본 글은 특정 돌봄 대상자에 대한 편견과 선입견을 허물고 보다 윤리적 간호 실천을 위한 수단으로 후반에는 하이데거와 가다머의 해석학을 논의한다. 해석학적 접근을 통한 윤리로 간호사는 돌봄 대상자의 존재에 대해 진정한 의미를 이해할 수 있으며, 이는 곧 상호작용 중심의 거대 간호이론과도 맞닿아 있다. 이 과정에서 간호사는 돌봄 대상자에 대한 편견, 즉 선이해를 깨고 궁극적으로 돌봄의 윤리를 재발견할 수 있어야 하겠다.
This paper reflects on the use of specific signs in nursing practice as a means of identifying difficult patients or difficult caregivers, with the aim of envisioning an ethical nursing praxis. Extending from normative ethical theories to a hermeneutic ethical framework, the paper examines how the signification of patients and caregivers through particular codes in clinical settings may foster prejudices and labeling effects that distort their Being and undermine the essence of nursing care. The discussion first reviews three major normative ethical theories, then turns to the hermeneutic ethics of Heidegger and Gadamer as a way to soften nurses’ prejudices and preunderstandings toward particular patients and caregivers. These Beings, represented through specific signs or codes, may disrupt the integration of nursing metaparadigms—person, health, environment, and nursing. By adopting an interpretive approach to the narratives of patients and caregivers, nurses may come to understand their existential meanings and rediscover the deeper ethical dimension of care, as articulated in grand nursing theories based on interactive processes.
