‘좋은’ 장애 돌봄이란 무엇인가 : 자폐성 장애 치료의 경험윤리 탐구

What Makes Disability Care ‘Good’?: Towards an Empirical Ethics in Autism Treatment

이 논문은 기술 집약적 치료를 포괄하는 확장된 돌봄 개념과 경험적 접근의 돌봄윤리 탐구 방식에 기반하여, 영유아기 자폐성 장애 치료의 현장에서 좋은 치료를 구성하는 다양한 가치들을 발굴한다. 자폐성 장애 돌봄에서 의료적 의사결정의 주체인 의사들과 보호자들은 자폐성 장애에 개입하는 치료 방식을 분류하고 선택하고 시도하는 가운데 상이한 가치를 지향하며 서로 다른 장애 돌봄의 방식을 형성한다. 의사들은 치료 효과의 과학적 입증 여부와 달성 가능성, 치료의 효율성을 핵심적 가치로 두고 치료를 처방함으로써 좋은 장애 돌봄을 추구한다. 반면, 보호자들은 자녀를 위해 적극적으로 다양한 치료 방식을 시도할 책임을 다하는 것을 중시하며, 개별 아동의 특성에 맞는 개별화된 치료를 추구한다. 또한 보호자들은 치료 과정의 즐거움, 치료가 아이로서의 일상과 정체성을 해치지 않는 것 등 치료와 일상의 조화를 좋은 장애 돌봄을 이루는 가치로 간주한다. 이러한 경험윤리 연구는 좋은 장애 돌봄의 형태가 다양할 수 있음을 보여주는 동시에, 더 좋은 장애 돌봄의 기술을 만들기 위한 논의의 장을 새롭게 열 수 있다.

Grounded in an expanded concept of care and an empirical approach to the ethics of care, this study examines the diverse values and logics of care that constitute ‘good treatment’ in autism care. Medical professionals and parents as primary caregivers involved in autism treatment have different normative approaches to medical decision-making. Medical professionals tend to value scientific evidence of treatment efficacy and the predictability of outcomes based on medical reasoning. They pursue rational medical decision-making by prescribing efficient treatments. In contrast, parents emphasize their moral responsibility to actively explore and try various treatment options for their children, seeking individualized care tailored to each child’s unique characteristics. Furthermore, parents value positive emotional responses from the child and the harmony between therapeutic practices and the child’s everyday life. By illuminating these differing yet coexisting values, this empirical ethics analysis demonstrates that ‘good care’ can manifest in various ways. It concludes that recognizing and negotiating such plural forms of goodness is essential for developing more inclusive and context-sensitive care practices.
