(Background and Purpose)According to the 2024 report by Statistics Korea, South Korea's elderly population (aged 65 and older) accounts for 19.2% of the total population and is projected to exceed 40% by 2050. The transition into a super-aged society, coupled with global challenges such as climate change, economic downturns, widening inequality, and low birth rates, has become a critical issue. Elderly individuals require various services, including disease prevention, health promotion, medical treatment, long-term care, and welfare, depending on their functional status. From a long-term perspective, addressing these complex demands requires a more comprehensive and integrated role for healthcare services. This study aims to serve as a foundational research effort for improving healthcare services by analyzing and comparing domestic and international healthcare service processes from a service design perspective.(Method)This study analyzes healthcare services from a service design perspective, using domestic and international case studies. First, a theoretical review is conducted to examine the concepts and characteristics of healthcare services and service design. Second, healthcare service categories are classified and structured according to design area, through which healthcare service design is derived. Third, these healthcare service design models are categorized based on design thinking principles to propose a systematic healthcare service design process. Finally, a framework based on the proposed healthcare service design process is used to analyze case studies from various countries, suggesting the potential applicability of this process and future directions for healthcare service development.(Results)The relevance of healthcare service design varies depending on the type of medical institutions or healthcare-related facilities, indicating that the significance of roles within healthcare service stages differs according to the institutional characteristics. Additionally, access to healthcare services varies across countries based on the type of healthcare coverage. Furthermore, innovative organizations composed of various stakeholders play a crucial role in enhancing healthcare service experiences.(Conclusions)This study recognizes the “wicked problems” in our society and, based on this awareness, classifies healthcare services from a long-term perspective analyzes case studies through a design thinking. Despite variations in healthcare coverage across countries, the study highlights the necessity and feasibility of establishing a community-centered healthcare system based on primary care, which actively engages local communities and facilitates participation. This system functions as a health promotion model that improves the well-being of individuals, families, and communities, emphasizing its necessity as a public service model. Additionally, this study suggests further strategic refinement of the healthcare service design process and recommends its expansion to research on healthcare service design processes tailored to national healthcare delivery systems.
